Position Summary:

The Assistant Care Manager supports the Care Manager in coordinating high-quality, compassionate, non-medical home care services. This position assists with client care coordination, caregiver supervision, scheduling, documentation, training, and communication with clients, families, caregivers, healthcare providers, and payer sources.
The Assistant Care Manager also serves as a role model and mentor for agency caregivers. Although this is not a regularly scheduled direct-care position, the Assistant Care Manager must be capable of providing services in a client’s home during emergencies or other urgent situations. Temporary direct-care coverage may also be assigned when a significant scheduling gap cannot be filled until an appropriate caregiver is hired or assigned. The Assistant Care Manager works under the direction of the Care Manager and does not independently assume full responsibility for a Care Manager’s caseload unless specifically assigned and authorized as their designee.
Requirements:Key Responsibilities:
· Assist the Care Manager with coordinating services for an assigned client caseload.
· Assist in client intake, service initiation, ongoing service coordination, reassessments, and discharge processes.
· Assist with gathering information for client assessments and individualized care plans.
· Assist with client visits, quality-assurance contacts, and service observations as assigned.
· Consistently monitor client services and promptly communicate changes in a client’s condition, needs, preferences, environment, or availability to the Care Manager.
· Assist in ensuring that care plans, schedules, and caregiver assignments accurately reflect each client’s authorized services and current needs.
· Communicate professionally with clients, family members, authorized representatives, healthcare providers, payer sources, and other members of the client’s care team.
· Follow up on client concerns, service interruptions, missed visits, and other care-related issues as assigned.
· Assist in supervising and coaching agency and family caregivers.
· Ensure accurate, timely, and complete documentation of client-related activities and communications.
· Assist in supporting continuity of care by ensuring relevant information is communicated to the appropriate members of the care team.
· Escalate significant client concerns, incidents, complaints, or changes in condition to the Care Manager or other appropriate leader.
· Provide direct care during emergencies or significant staffing gaps.
· Participate in on-call rotation as assigned.
· Assist in maintaining accurate caregiver scheduling and service records.
· Follows all company policies, confidentiality requirements and regulatory standards.
· Participates in audits, quality-improvement and special projects as assigned.
· Participate in client and caregiver events as assigned.
· Fill in as Care Manager designee during Care Manager absences.
Qualifications:
· High school diploma or equivalent. Associates degree preferred.
· 2+ years of experience in home care, healthcare, human services, care coordination, client services, or a related field preferred. Experience in a supervisory, caregiver-coordination, or team-lead role is a plus.
· Previous leadership, mentoring, training or supervisory experience.
· Ability to provide emergency or temporary direct-care coverage.
· Ability to use artificial intelligence and scheduling systems.
· Ability to pass an extensive background check process.
· Bilingual in Spanish and English is required.
Additional Expectations:
· This position requires travel to clients’ homes, participation in assigned on-call rotation and occasional work outside normal business hours. Duties may change based on client, agency and regulatory needs. This position is an in-office position. Hours are Monday through Friday, 8:00am to 5:00pm with an hour’s unpaid lunch break.
· This position may require interim oversight of client and caregiver case load as business needs arise
