Job Description

ACT Medication Management Provider

Reports to: Medical Director

FLSA Status: Full-Time Exempt

Position Summary:

The ACT Medication Management Provider delivers psychiatric assessment and medication services to participants in The Well’s multidisciplinary Assertive Community Treatment (ACT) program. The provider collaborates with each participant and the ACT team to make timely, person-centered treatment decisions, monitor medication response and safety, and coordinate psychiatric care with physical health, substance use, housing, vocational, and recovery supports.

The position participates in ACT’s shared-caseload and daily meeting practices, provides services in community settings when clinically appropriate, and supports the program’s scheduled 24/7 crisis response through an assigned after-hours on-call rotation. The provider gives prescriber-level consultation and medication follow-up during assigned coverage, following The Well’s on-call protocols, professional scope, and emergency procedures.

Core Values:

  • Show Up with Generosity: You build authentic relationships through gratitude, partnership, and consistent communication.
  • Check the Ego: You collaborate closely with program and finance staff, valuing teamwork and accuracy over individual credit.
  • Embrace Diversity: You celebrate and honor the cultures, identities, and lived experiences of those we serve and ensure donor messaging reflects this shared value.
  • Grounded in Hope: You believe in the transformative power of community investment and bring optimism, professionalism, and persistence to your work.

Qualifications:

1. Education, Licensure & Experience

  • Graduate medical or nursing education appropriate to the provider’s license and scope. A psychiatrist, psychiatric mental health nurse practitioner, or another qualified prescribing clinician may serve in this role when authorized to provide the assigned psychiatric assessment and medication management in Nebraska.
  • Current, unrestricted Nebraska professional license and prescribing authority appropriate to assigned services. Maintain required certifications, agency privileges, and state or federal registrations for any medications prescribed, including controlled-substance registration when applicable.
  • Psychiatry or psychiatric mental health training and experience treating adults with serious mental illness preferred. Experience with ACT, community-based care, co-occurring substance use, and people with complex medical or social needs is preferred.

2. Clinical & Professional Skills

  • Demonstrates sound psychiatric judgment, medication knowledge, risk assessment, and ability to make shared treatment decisions with participants.
  • Can coordinate treatment across psychiatry, primary care, nursing, substance use services, hospitals, pharmacies, and community providers while protecting participant choice and confidentiality.
  • Communicates clearly with participants and staff, completes accurate records, and responds calmly to changing clinical needs and crisis consultation.
  • Uses trauma-informed, culturally responsive, recovery-oriented practice and maintains professional boundaries and ethical standards.

3. Other Requirements

  • Meet The Well’s job-related credentialing, background, screening, and fitness-for-duty requirements; maintain required professional liability coverage as applicable.
  • Ability to participate in flexible scheduling, community-based services, and scheduled ACT after-hours coverage.
  • Valid Nebraska driver’s license and insurable driving record when driving is required for assigned duties.

Duties and Responsibilities:

1. Psychiatric Assessment & Medication Management

  • Conduct psychiatric evaluations and follow-up assessments for ACT participants, including review of symptoms, functioning, medical history, substance use, current medications, treatment response, and safety concerns.
  • Develop and update medication plans with participants using shared decision-making. Prescribe, adjust, discontinue, and monitor medications only within the provider’s license, training, agency privileges, and applicable requirements.
  • Provide medication education and monitor effectiveness, adverse effects, interactions, adherence barriers, and participant preferences. Coordinate laboratory testing, vital signs, physical health screening, and medical follow-up with ACT nursing and medical providers as appropriate.
  • Support access to and continuity of psychiatric medications, including coordination with pharmacies, hospitals, residential programs, and other prescribers during transitions of care.
  • When qualified and clinically indicated, assess and prescribe medications for substance use disorders in coordination with the SUD Specialist and participant’s broader treatment plan.

2. ACT Service Delivery & Shared Caseload

  • Participate in daily ACT meetings and shared-caseload reviews. Help the team understand medication-related needs, clinical risks, pending tests, follow-up tasks, and changes in participant status.
  • Provide psychiatric services in the clinic and, when clinically appropriate, in participants’ homes or other community settings. Coordinate with the ACT Team Lead to make services timely and accessible.
  • Contribute to individualized treatment, rehabilitation, recovery, and crisis plans. Review plans with participants and the team at required intervals and when clinical needs change.
  • Consult with the RN, ACT Team Lead, clinicians, Case Manager, SUD Specialist, Peer Specialist, Vocational Specialist, and other team members to coordinate integrated care and reduce gaps during hospital, jail, or residential transitions.

3. After-Hours On-Call & Crisis Coverage

  • Participate in the scheduled ACT 24/7 on-call rotation, including evenings, weekends, and holidays as assigned. During assigned coverage, provide timely psychiatric and medication consultation for ACT participants and support the team’s crisis response.
  • Respond to after-hours calls using The Well’s protocols: review available clinical information, assess urgency, advise ACT staff, arrange clinically appropriate medication follow-up, and coordinate with the RN, ACT Team Lead, CCBHC/mobile-crisis resources, or emergency services as needed.
  • Make prescribing or medication decisions only when clinically appropriate and within professional scope, available information, agency privileges, and applicable law. Escalate situations requiring urgent in-person assessment, emergency department evaluation, or a higher level of care.
  • Document on-call contacts, clinical advice, medication decisions, and follow-up plans promptly in the EHR or approved on-call record, and hand off unresolved needs to the team.

4. Team Consultation, Training & Quality

  • Provide consultation to ACT staff about psychiatric symptoms, medication concerns, side effects, adherence barriers, and coordination with other treating providers.
  • Contribute to ACT orientation and role-specific staff training on psychiatric medications, medication safety, recognizing urgent symptoms, and escalation procedures, as assigned.
  • Participate in case reviews, supervision or consultation, TMACT fidelity preparation, clinical quality improvement, and review of incidents or medication-related concerns.

5. Documentation, Grant & Compliance Responsibilities

  • Complete timely, accurate documentation of psychiatric assessments, diagnoses, medication orders, informed consent or shared decision-making, coordination, on-call contacts, and clinical follow-up in the EHR.
  • Complete required authorizations, medical necessity documentation, treatment plan contributions, and payer or agency records within required timeframes.
  • Provide accurate ACT service information for grant reporting with the Project Director, evaluator, and Program Assistant, including medication services, after-hours response, and care transitions.
  • Support ACT adherence to Nebraska requirements, applicable CCBHC and payer standards, TMACT fidelity expectations, The Well policies, HIPAA, 42 CFR Part 2, professional ethics, and approved grant conditions.
  • Accurately document grant-related effort and services according to approved funding allocations. Do not charge the same time or service to more than one funding source.

6. The Well Responsibilities

  • Follow The Well’s policies for attendance, timekeeping, confidentiality, participant rights, safety, incident reporting, and professional conduct.
  • Complete assigned orientation, training, meetings, credentialing, performance reviews, and continuing education; maintain required licenses and registrations.
  • Represent The Well professionally with participants, families, coworkers, pharmacies, and community providers. Perform other related duties consistent with the position and professional scope.
