Home Health Registered Nurse (RN)
The Home Health Registered Nurse Home Health RN who will be responsible for the care of patients in their home. Our service area is Plymouth County, Iowa. The qualified candidate will have 2 years of experience performing OASIS assessments and care planning for in-home patients. Previous experience with our Epic is a plus. This person will also be in the call rotation for the department, one in four nights, weekends and holidays. Scheduled shifts are days, Monday through Friday. We are open to 32-40 hours per week.

Responsibilities
Assess patients and monitor changes in conditions.
Administer medications, IV therapies, and treatments.
Develop and update individualized care plans.
Provide wound, catheter, preoperative, and postoperative care.
Educate patients and families.
Coordinate care with the interdisciplinary team.
Complete, timely and accurate documentation.
Follow all patient-safety and infection-prevention standards.

Qualifications
Active RN license for the state of practice.
Current American Heart Association BLS certification.
ACLS certification if required by the facility.
Minimum of 3 years of recent Medical-Surgical experience.
Strong clinical judgment, communication, and time-management skills.
Level I Hospital Experience preferred/some units required
EMR: Epic experience preferred
