Position Summary

The Resident Care Manager/Nurse Manager (RN) is responsible for overseeing and coordinating resident care plans to ensure the highest quality clinical outcomes and person-centered care. This role serves as a key clinical leader within the interdisciplinary team, supporting nursing staff, collaborating with healthcare providers, and helping create a positive and efficient care environment for both residents and employees.

The ideal candidate is a strong communicator, an organized leader, and a team-oriented professional who thrives in a fast-paced healthcare setting and is passionate about supporting both residents and fellow staff members.

Essential Responsibilities

• Coordinate, develop, and oversee individualized resident care plans

• Conduct comprehensive assessments for new admissions and current residents

• Collaborate closely with interdisciplinary teams to ensure quality resident outcomes

• Monitor resident conditions and communicate changes appropriately using clinical best practices

• Review physician orders, test results, and treatment plans to ensure timely follow-through

• Maintain accurate and compliant clinical documentation within the electronic medical record system

• Participate actively in clinical meetings and care planning discussions

• Support and mentor nursing staff while promoting teamwork and accountability

• Help ensure smooth care transitions and discharge planning coordination

• Promote excellent customer service and positive communication among staff, residents, and healthcare partners

• Maintain compliance with facility policies, professional standards, and state/federal regulations

Qualifications

• Education & Licensure: Graduate of an accredited nursing program; current Registered Nurse (RN) license and CPR certification required.

• Experience: Previous experience in long-term care or skilled nursing preferred. Minimum of one (1) year supervisory or leadership experience in a healthcare setting preferred.

• Skills & Attributes: Strong leadership and organizational skills, excellent communication abilities, passion for leading and supporting team members, and knowledge of nursing practices and regulations within skilled nursing care.
