Job Summary:

As part of the Waseca County Collaborative for Families (WCCF) team, the Family Service Coordinator (FSC) provides individual and family services for children. The Family Service Coordinator serves as the liaison between school, family, and other community resources. The FSC provides services as needed in the school, home, and community setting. The focus of the FSC is on prevention and early intervention that is supported through employment placement within the elementary school setting

Supervisory Responsibilities:

Duties/Responsibilities:

  • Provide individual and family services for children, serving as the liaison between school, family, and other community resources.
  • Provide services as needed in the school, home, and community setting.
  • Maintain documentation of interventions and student services.
  • Counsel and provide skills training to students individually and in small groups.
  • Refer students and/or family members to appropriate school and community resources/services.
  • Develop an ongoing working relationship with county and regional service providers.
  • Attend monthly FSC staff supervision meetings.
  • Develop a strong working relationship with school staff and assist in identifying, documenting and problem-solving at-risk behavior patterns among students.
  • Inform and educate school staff on the FSC referral process for individual and group services.
  • Contact families of students who have been identified as at-risk and visit their homes as appropriate.
  • Counsel families of students who have been identified as at-risk on resources/services.
  • Document referrals and services provided and track numbers for WCCF program evaluation and end of the year reporting.
  • Work with WCCF to bring about positive systemic change in the service delivery system in Waseca County.
  • Help develop prevention strategies and creative systems that will support at-risk students and their families.
  • Take in-service training courses as required for the position and maintain licensure when required.
  • Act as the school’s McKinney-Vento representative.
Requirements:

Required Knowledge, Skills, and Abilities:

  • Excellent interpersonal communication skills; deal effectively with a variety of people and personalities; communicates clearly and effectively both orally and in writing.
  • Works as an effective and proactive team-player.
  • Able to work independently, showing initiative, and willingness to take charge of a situation.
  • Skilled in identifying and assessing a wide range of clients’ needs.
  • Knowledgeable about local and regional community resources and how to access them.
  • Applies effective time management skills to meet all deadlines.
  • Demonstrates thoroughness and organizational skills in the workplace.
  • Demonstrates the ability to organize meetings with clients, families, referral sources, etc. for consultation and collaboration.
  • Understanding of community funding resources for client services, including housing, basic needs, special services, medical and mental health care, benefits assistance, etc.
  • Staff will have knowledge of child developmental stages and child mental health diagnosis.
  • Demonstrates financial prudence with funding resources.

Education and Experience:

  • MINIMUM QUALIFICATIONS: Bachelor’s degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology or nursing from an accredited college or university.
  • Previous experience working with children experiencing mental health or behavioral difficulties providing individual or group services.
  • Previous experience of working with various types of family systems.
  • Prior knowledge of community resources for children and families.

Physical Requirements:

  • Must be able to transport clients in the community.
  • This position requires advanced mobility, including but not limited to frequent walking.
  • Ability to lift up to 15 pounds.

Work Environment:

  • Align with the school's culture to become integrated within the school building.
  • Maintain regular work hours as determined and assigned
  • Perform miscellaneous projects and complete various tasks as requested by school administration.
  • Understands the confidential nature of organizational information and maintains confidence.
  • Work tasks may be completed in the client’s home, various community locations or within or outside of the school setting.
