Clinical Social Worker in Brockton, Massachusetts at Signature Healthcare

The clinical social worker, as part of the integrated behavioral health team within the SMG Patient Centered Medical Home, engages in a collaborative care process with primary care providers who assess, plan, implement, coordinate and monitor options and services to meet a patient's comprehensive healthcare needs. The social worker has experience in ambulatory primary care medical setting, evidence-based practice, and ambulatory social work billing practices. The social worker identifies appropriate behavioral health care and other community resources to meet patient needs and optimize patient experience. Experience in working with individuals and families related to co-occurring behavioral health and substance use disorders is a plus. The social worker will demonstrate cultural humility, safety and responsiveness and address inequity/power imbalances in all patient and professional interactions. The social worker will research and collaborate with social work and community partners to locate concrete services for patients.

Masters in Social Work or Masters in Mental Health Counseling required. The job requires a license of Clinical Social Worker, Licensed Independent Social Worker, or Licensed Mental Health Counselor.

Ambulatory Social Work is based at 110 Liberty St., Brockton, MA. This role will be covering a team member absence based at 1 Donald's Way, E. Bridgewater, MA from June through December 2026. Position reports to the Integrated Behavioral Health Social Work Supervisor.
