Shift Availability for Care Navigator:

• Monday-Friday
• Dayshift

Responsibilities of Care Navigator:

• Guide Transitions: Help residents and families navigate the complexities of moving between levels of care (Residential Living to Personal Care or Skilled Nursing) with grace.
• Coordinate Wellness: Assist with home-care arrangements, and partner with our rehab team.
• Advocate & Educate: Assist residents in understanding advance directives, insurance transitions, and community resource options.
• Bridge the Gap: Act as the vital link between hospital staff, families, and our internal clinical team during hospitalizations or health changes.
