Purpose of the Positions:
The Hospice Registered Nurse is a key member of the interdisciplinary team. The hospice nurse evaluates clients’ physical, emotional, psychosocial, and spiritual needs and develops appropriate care plans and interventions accordingly. The nurse is the case manager and oversees care to ensure the highest quality of care is maintained.
Essential Responsibilities:
- Coordinates the care of the client and services, which may include the social worker, the chaplain, an in-home aide, the physician, and/or a volunteer
- Assesses the client and family within 24 hours of admission to Hospice
- Provides skilled nursing care as determined by the IDG plan of care.
- Provides education to clients, families, and/or others in care techniques specific to the client
- Serves as client/family advocate by maintaining regular communication with physicians and any other resources when needed
- Participates in providing bereavement support to clients and families
- Documents and ensures appropriate use of medications, DME, and supplies
- Participates in weekly meetings and IDG meetings
- Provides an appropriate care plan and leaves in the home for other staff members and contract staff personnel
- Monitors the recertification/reauthorization process
- Accurately documents and follows through with the revocation process
- Reports regularly to the Hospice Administrator, client updates, and reports status
- Maintains confidentiality in all aspects of the job
- Promotes personal safety and a safe environment for clients and co-workers
- May complete In-Home Aide annual performance appraisals and assist with yearly competency testing
- Participates in on-call rotation
- Participates in performance improvement initiatives as requested
- May participate in the orientation of new staff
- May act as a preceptor for new staff
- Represents the nursing profession in a positive manner
- May participate in community educational activities as needed
- Promotes and participates in professional development for nursing and palliative care
- Performs additional duties as requested
- On-call rotation
- Covering counties to include: Bladen, Cumberland, Harnett, Hoke, Lee, Robeson, Sampson, Scotland, and surrounding counties.
Qualifications:
- Active NC nursing license
- Function as a core member of the IDG
- Ability to speak in front of groups
- Maintain a high-quality customer service approach
- ADN required
- At least one year of hospice or home health nursing experience preferred
- Possess active listening and excellent assessment skills
- Working knowledge of a variety of computer programs is desired
Physical Demands:
Ability to sit for long periods, either while driving, participating in IDG meetings, visiting clients, or performing computer work. This position may require frequent bending, reaching, lifting, talking, and listening.
