Job Summary:
The Physical Therapist (PT) provides skilled evaluation, treatment, and care planning for patients in the home setting under physician orders. The PT is responsible for restoring function, reducing pain, and promoting safe, independent mobility while ensuring compliance with all state, federal, and accreditation standards.

Essential Functions
- Perform comprehensive evaluations, including OASIS assessments, within required timeframes
- Assess mobility, strength, balance, pain, endurance, and safety risks in the home
- Identify functional limitations and establish measurable, patient-centered goals
- Develop, implement, and update individualized plans of care in collaboration with physicians, patients, and the interdisciplinary team
- Obtain and manage physician orders for therapy services and plan updates
- Determine need for continuation, modification, or discharge of therapy services
- Deliver evidence-based physical therapy interventions based on patient diagnosis and condition
- Monitor patient response and adjust treatment plans as appropriate
- Educate patients and caregivers on home exercise programs, pain management, safe mobility, and fall prevention
- Communicate changes in patient condition to physicians and care team members
- Participate in care coordination and case conferences
- Complete accurate, timely EMR documentation, including evaluations, visit notes, OASIS, and discharge summaries
- Ensure documentation supports skilled need, progress, and regulatory compliance
- Supervise PTAs in accordance with regulations and agency policy
- Participate in training, in-services, and quality improvement activities
- Utilize community resources to support patient care and discharge planning
- Graduate of an accredited physical therapy program.
- Current, unrestricted Colorado PT license.
- Minimum of one year of experience as a PT in acute care, rehabilitation or skilled setting. Prior home health experience preferred.
- Maintains patient confidentiality and HIPAA compliance at all times.
- Valid driver’s license, reliable transportation, and current auto insurance.
Physical Requirements
- Ability to stand, walk, bend, and lift up to 50 pounds.
- Ability to assist with patient mobility and transfers in varied home environments.
