About This Position

Provide comprehensive outpatient counseling and assessment services to children and adults presenting with mental health diagnoses, who may also present with AODA issues.
  1. Ensure adherence with Clinic, CCS and CSP regulations that there are written orders on file for any prescription medications.
  2. Ensure that all medication monitored by the Clinic, CCS and CSP has a label permanently attached to the outside of the container, which identifies its contents. Ensure all aspects of medication handling are in accordance with CCS and CSP requirements. Monitor quality assurance related to medication monitoring.
  3. Set up medication trays.
  4. Administer IM medication to consumers. Injections generally take place in the clinic but may take place in the consumer's home unless it is unsafe or unfeasible, which is decided on a case-by-case basis.
  5. Educate staff on issues of health and safety.
  6. Educate staff of medication side effects and medication monitoring. Provide training to new staff on appropriate policies and procedures for medication monitoring.
  7. Develop and implement strategies for providing consumer education regarding medical issues.
  8. Notify case managers and therapists of health related documentation needed for consumer records.
  9. Maintain written records and documentation in accordance with Clinic, CCS and CSP regulations.
  10. Participate in the formulation of treatment plans, as appropriate.
  11. Notify the case managers or therapists of any psychiatric symptoms or behavior changes that may require intervention.
  12. Make recommendations for educational and clinical changes to the Clinical Director.
  13. Maintain regular oral and written communication with program supervisors regarding job duties, concerns and program status reports.
  14. Attend and participate in staff meetings and in-service trainings.
  15. Liaison with community medical professionals.
  16. Participate in professional staffing as required by license.
  17. Participate in continuous program quality-improvement efforts, which emphasize treatment engagement, continuation in treatment and successful completion of treatment.
  18. Utilizing electronic case management software (TCM – The Clinic Manager) record summary of each encounter, including notations of missed, cancelled appointments, as well as telephone contacts according to Medicare, Medicaid and other clinic standards.
  19. Maintain a professional and confidential relationship with patients.
  20. Employee will maintain records that are in compliance with Racine County and Wisconsin regulations.
  21. Employee will maintain required production levels.
  22. Provide oral and written communication that is appropriate to the task and in accordance with agency, DHS 35, 36, 75 and 63 and Medicaid standards.
  23. Back up for the CSP nurse and with CCS consumers as needed.
  24. Communicate pertinent information on a timely basis and participate in clinical supervision case reviews, staffing's, and/or case planning meetings.
  25. Complete all necessary health care/medical and financial record documentation required to maintain complete medical and financial billing records.
  26. Transport medication to individual sites or consumers, as necessary.
  27. Perform medical / psychiatric outreach, as needed.
  28. Prepares charts and/or information and consumer updates for the prescribers.
  29. Scheduling follow-up appointments for the prescribers.
  30. Signs medication consents, treatment plans and treatment consents with consumers.
  31. Maintain supply cabinet.
  32. Answer calls either transferred to the nurse or on the med line.
  33. Order supplies and injections needed.
  34. Maintain updated injection schedule.
  35. Complete prior authorizations as needed.
  36. Perform other duties as assigned, including responding to an emergency event.

Supervision Received
Receives supervision from the Clinical Director.
  • A registered nurse with current certificate of registration under Ch. 441. Preference for a Bachelor's Degree in Nursing and/or certification as a generalist on psychiatric nursing is recommended but not required.
  • 2 years of experience preferred.
  • Valid Wisconsin driver's license and automobile insurance are required.
  • Prompt and regular attendance.

  • Must be a Registered Nurse with a current certificate of registration under Chapter DHS 441.
  • Must demonstrate clinical competence in standards of nursing practice and ability to work independently in area of expertise.
  • Must have ability to communicate effectively orally and in writing, and interact effectively with consumers and other health care professionals.
  • Must own and be able to legally operate a vehicle and have necessary insurance.
  • Must be able to lift and carry 20 pounds, climb two flights of stairs and use public transportation.
  • Must be able to assist consumers with activities of daily living, such as shopping, cleaning and cooking.
  • Ability to communicate effectively and professionally using written, verbal or electronic formats.
  • Ability to maintain excellent relationships with members served and community partners
  • Familiarity with a variety of evidence based practices appropriate to members served
  • Working knowledge and experience with outpatient services including DHS 35, 75, Patient Rights, 42 CFR and all regulations governing State of Wisconsin
  • Ability to develop and maintain an effective body of knowledge regarding BHS responsibilities to the community and consumers served.
  • Ability to work effectively with multi-disciplinary team.
  • Ability to relate to diverse economic and ethic population
  • Ability to work independently, yet fully participate as part of a therapeutic clinical team of professionals including mental health and substance abuse therapists, psychiatrists, APNPs and nurses.
  • Excellent written and oral communication skills.
  • Ability to solve problems and negotiate solutions
  • Technically proficient with Microsoft office products, internet browsing and EMR.
  • Interacts with the public in a professional manner.
  • Must be able and willing to work evenings.
