Multiple Sclerosis RN Navigator
Position Summary:
Assists patients in understanding their multiple sclerosis diagnosis, treatment options and the resources available.
Responsibilities:
- Serves as an essential link between patients and all other medical staff
- Facilitates appointments for consults and support services within established service standards
- Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines
- Triages symptoms and coordinates with physician and other health care providers on appropriate intervention and symptom management
- Manages complex outbound calls requiring RN level assessment, clinical judgement, teaching, and self-management support (such as abnormal, complex test results, medication changes, etc.)
- Discusses disease management, symptom management, and reinforces medical treatment plan for MS
- Reviews and discusses medications in depth and provides in-office injection training
- Monitors response to medication change and response to interventions
- Works closely with specialty pharmacists for coordination of care with disease modifying therapies
- Develops patient education programs and tools
- Follows patients throughout the care continuum, including inpatient admissions and collaborates with inpatient care management resources
- Maintains knowledge of financial resource assistance with imaging, exercise equipment, copays, therapies, etc.
- Educates patients on community resources and mitigates barriers to care
- Promotes participation in the workforce for as long as possible by providing assistance with accommodation and leave applications and advocates for the other needs of the patient (child care, housing, transportation, etc.)
- Adheres to established productivity benchmarks; identifies opportunities to reduce the cost of care
- Performs other related duties as assigned
- Familiar with standard concepts, practices, and procedures within the field
- Relies on experience and judgment to plan and accomplish goals
- Regular and predictable attendance is an essential job function
- Competent to meet age specific needs of the unit assigned
Requirements and Minimum Qualifications:
- BSN required; MSN preferred
- Graduate of a nationally accredited nursing program
- Licensed as a RN by the Idaho Board of Nursing OR licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the "multi-state privilege to practice" compact with Idaho
- Minimum of 2 years of multiple sclerosis nursing experience required
- Multiple Sclerosis Certified Specialist certification required within one year of meeting eligibility requirements and maintains competency by completing continuing education credits related to MS
- Must complete KH BLS during orientation period
