About This Position
Affinity Healthcare Solutions Skilled Care, LLC is a CHAP-accredited home health agency providing skilled and non-skilled care throughout Pennsylvania. Our team is committed to delivering compassionate, high-quality care that empowers clients to live independently and safely in the comfort of their homes.
We are seeking dependable, compassionate, and caring Certifed Home Health Aides (HHAs)/Certified Nursing Assistants—also known as Care Ambassadors—to join our growing team.
Responsibilities
Provide personal care in coordination with the client’s skilled nursing plan of care
Assist clients with bathing, grooming, hygiene, and mobility needs
Perform light housekeeping and meal preparation as directed
Monitor and report client condition changes promptly to the supervising nurse
Accurately document all visits in accordance with agency policy and EMR guidelines
Ensure patient safety and dignity while delivering care in compliance with agency and CHAP standards
Qualifications
Active HHA certificate or CNA Certificate
Valid CPR certification
Recent physical and 2-step TB test or chest X-ray
Reliable transportation and valid driver’s license
Must pass background and reference checks
Prior experience with skilled or post-acute clients preferred
Strong communication and time management skills
