Shifts: Days, Evenings, Nights, and Weekends

C-Care Health Services is an accredited healthcare staffing and home care agency with over 20 years of experience providing professional, compassionate, and quality care services across the Greater Toronto Area.

We are currently seeking experienced, reliable, and dedicated Personal Support Workers (PSWs) for hospital assignments in Downtown Toronto, including constant patient observation and floor duty.

We are looking for individuals who demonstrate professionalism, strong patient safety awareness, and a commitment to providing compassionate care in hospital environments.

• Provide constant observation and one-on-one supervision of patients requiring close monitoring.
• Monitor patients at risk of falls, wandering, confusion, or other safety concerns.
• Perform hospital floor duty assignments, assisting patients and nursing staff as directed.
• Assist patients with activities of daily living, including feeding, toileting, grooming, and personal hygiene.
• Support patient mobility, safe transfers, repositioning, and fall prevention.
• Observe and promptly report changes in patients' conditions, behaviours, or safety concerns to nursing staff.
• Follow hospital policies, infection prevention protocols, and patient safety procedures.
• Maintain accurate and timely shift documentation.

Required:

• Personal Support Worker (PSW) certificate from a recognized Ontario college or an equivalent, accepted PSW training program.
• Minimum one year of relevant patient care experience.
• Valid CPR/BLS certification.
• Strong communication and interpersonal skills.
• Ability to remain alert, attentive, and professional throughout assigned shifts.
• Reliable attendance, punctuality, and a strong sense of responsibility.
• Ability to commute reliably to Downtown Toronto hospitals.

Preferred:

• Previous experience in hospital settings, including constant observation or floor duty.
• Experience supporting patients with dementia, cognitive impairment, or high fall risks.
• Consistent availability, including evenings, nights, and weekends.

• Work with an established and accredited healthcare agency.
