About This Position

The ULTIMATE community connector role. YOU help families navigate the system so they can thrive!

This position is hands-on in the community, assisting individuals, families, and groups in accessing vital resources like health insurance, food, housing, and quality care information. You’ll work directly with families on electronic applications for AHCCCS (Health-e-Arizona PLUS), SNAP (formerly Food Stamps), and make referrals to other community resources. You will also guide people through understanding, applying, and enrolling in health coverage through the Marketplace. On top of that, you’ll facilitate health education, screenings, and workshops to inspire positive behavior changes in families and individuals.

HOW WILL YOU MAKE AN IMPACT IN THIS ROLE?

You will serve as a trusted resource for our communities, guiding them through critical programs and resources. By providing personalized assistance with health insurance enrollment, food assistance, and other community services, you ensure that families and individuals can access what they need to thrive. You will also build relationships, conduct outreach, and facilitate educational workshops to increase community awareness and engagement. Your work will directly support our mission to improve access to care and resources for those we serve.

MINIMUM QUALIFICATIONS AND COMPETENCIES

Education / Background:

  • High School Diploma/GED required; Bachelor’s Degree preferred in Community Outreach, Healthcare, Social Work, or related field.
  • 2+ years of experience in healthcare, community outreach, or related field.

Skills and Requirements:

  • Fluent in English and Spanish, with the ability to easily transition between both languages.
  • Knowledge of local population demographics, assets, and needs.
  • Familiarity with health insurance programs including Medicaid, KidsCare, and the Marketplace.
