About This Position

The HCBS Team Lead provides both leadership support to the HCBS department and direct care to assist persons served in reaching their maximum. The HCBS Team Lead will support HCBS department. This is a full-time position, 40 hours per week, flexible schedule including all shifts/weekends/on-call. To perform this job successfully, an individual must be able to perform each essential function satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the primary job responsibilities. The HCBS Team Lead reports to the HCBS Coordinator.

  • Coordinates, leads, or attends PSCP/interdisciplinary meetings, assist in the development of written programs, and ensure correct implementation of written programs.
  • Process intake paperwork.
  • Participate in the shared responsibility of HCBS audits and surveys
  • Assist with organizational on-call duties, and cover shifts as necessary.
  • Responsible for communicating appropriate persons served information to staff (i.e. social history, programming, medical needs, and other appropriate information to provide quality care).
  • Coordinates family services by setting up services, assisting with admissions and discharge, recordkeeping, correspondence, communication with case managers, and participating in conferences.
  • Address issues that arise in a manner that adheres to OU’s policies, guidelines, practices, and procedures
  • Ensure that all organizational, local, state and federal program guideline are being met and followed.
  • Maintain a clean and safe environment.
  • Attend required and assigned staff meetings and trainings.
  • Maintain and promote positive relationships with coworkers, persons served, families, vendors, and volunteers.
  • Contribute to team effort by accomplishing assigned tasks outside of home department.
  • Participate on cross- functional teams.
  • Advise appropriate supervisor/manager/director of any issues to ensure efficient operations and excellent service to all persons served.
  • Other duties as assigned by leadership.
