Salary: Starting at $61,000-$78,000 annually based on experience.

Statement of Purpose: The Clinical Quality Improvement Coordinator (CQAC) is an advocate for both the patient and for CAN Community Health. Ultimately, the goal is to enhance the quality of healthcare services provided at CAN to meet or exceed established industry standards as well as CAN’s exacting quality requirements. The CQIC utilizes analyses from reports, audits, and other feedback from payers and grantors to continually improve clinical documentation and data captured therein.

Essential Duties:

  1. Develops and implements a system for conveying quality measure performance to individual providers from all payers.
  2. Educates clinicians on best practices to enhance all aspects of quality outcomes.
  3. Identifies opportunities to improve quality measures and gaps in care progress.
  4. Assists with workflow design and redesign to promote clinical quality improvement.
  5. Develops and implements comprehensive clinical quality improvement policies and procedures.
  6. Collaborates with stakeholders to align clinical quality improvement objectives and initiatives.
  7. Collects and compiles data for special projects as requested for quality improvement purposes by payers, leaders, and committees.
  8. Participates in external payer audits and contributes quality improvement data as requested.
  9. Leads monthly continuous quality improvement meetings for each clinic and collects quality improvement data as required by payers, grantors, and internal committees.
  10. Represents CAN on quality management meetings with various grantor jurisdictions.
  11. Works with EHR Team on clinical quality measure dashboards and implementing tools for optimizing electronic data capture.
  12. Works with the Ryan White Program Team to create and maintain quality management plans.
  13. Works with Clinical Quality Control and Clinical Quality Assurance personnel to encourage continual quality improvement within the department and across the organization.
  14. Maintains professional manner in all aspects of job tasks.
  15. Demonstrates courtesy, compassion, and respect in all interpersonal relationships with CAN patients, clients, staff, vendors, and visitors.
  16. Adheres to ethical and legal standards of professional practice.
  17. Protects organization’s value by keeping information confidential in accordance with HIPAA, professional, and departmental standards.
  18. Promotes and practices CAN’s mission and values and follows its policies and procedures.

Secondary Duties:

  1. Continues in ongoing professional training and education to advance knowledge of quality measures and payer requirements.
  2. Utilizes resources and supplies in a cost-effective manner.
  3. Continues in ongoing professional training and education to advance knowledge of value-based care concepts and practices.

Knowledge, Skills, and Abilities:

  1. Demonstrates knowledge of electronic health records (eCW preferred).
  2. Demonstrates knowledge of quality measures and payor requirements.
  3. Demonstrates knowledge of quality improvement concepts and practices.
  4. Demonstrates knowledge of clinical documentation protocols, medical terminology, and clinical processes.
  5. Demonstrates ability to self-motivate.
  6. Demonstrates strong communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  7. Demonstrates ability to coordinate numerous tasks as required.
  8. Possesses strong problem-solving skills.
  9. Possesses ability to present to groups in-person or via electronic systems.
  10. Maintains ability to travel up to 20% to CAN clinics to conduct and support onsite audits.

Education/Professional:

  1. Graduate of an accredited school of nursing with active, unencumbered RN license in a state where CAN operates (preferred), or
  2. Bachelor’s degree in healthcare or related field
  3. 2 years of clinical experience
  4. Quality improvement experience in a clinical setting preferred
  5. Certified Professional in Healthcare Quality certification recommended

Physical Demands:

  1. Neat professional appearance.
  2. Exerts up to 10 lbs. of force occasionally and /or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects. Involves sitting most of the time but may involve walking or standing for brief periods.
  3. Requires expressing or exchanging ideas by means of spoken word, written word, visual and auditory acuity.

Must be able to pass a Level I background check (a Level II background may also be required).
