POSITION SUMMARY/RESPONSIBILITIES
The Coordination of Care Project Manager (CCPM) serves as the liaison between the Coordination of Care department to the outpatient clinics, ancillary departments and the hospital staff. The CCPM assesses and improves department performance, and ensures orientation and continuing education of departmental staff. The CCPM implements projects and strategies to attain and exceed established departmental goals and metrics.
EDUCATION/EXPERIENCE
Bachelor’s degree in Business Administration, preferred. In lieu of a degree, a minimum 5 years of experience in care coordination is required. Working knowledge of Patient Navigation, Patient Care Coordination, Hospital Discharge Workflows, Population Health Management, familiarity with all metric dashboards and Durable Medical Equipment and Home Health processes are required. Competence with Electronic Medical Records (EMR) systems and familiarity with EMR Work Queues is required. Strong organizational and interpersonal skills. The ability to work in a high pressure or changing environment and exercise good judgment to resolve problems in the absence of formalized procedures and guidelines. Self-starter and ability to work independently. Prior leadership experience is preferred.
