Job Description
Job Summary:
Promotes customer satisfaction by providing courteous, confidential service through competent, professional practices and prompt response as outlined in the patient focused care affirmation.
- Adheres to the patient focused care agreement, acts as the change agent, presents a professional approach and appearance, a friendly smile and pleasant tone of voice, operates as an active part of the team, uses constructive problem solving, maintains confidentiality, promotes a positive image through an efficient and cooperative approach to all tasks, refrains from criticism of co-workers, the department, hospital, guests and patients.
- Interacts with requestors to identify medical records needed for patient care, review, billing or release of information.
- Prioritizes release of information requests, retrieves medical records from active and inactive and interdepartmental locations, electronically tracks and delivers records in accordance with established procedures.
- Performs self-quality checks on all work to assure accuracy of the release, confidentiality of proper invoicing.
- Handles all requests and inquiries for patient health information whether received via mail, fax, and phone or in person.
- Verifies the patient identity and confirms that the authorization is valid
- Ensures the requesting party has a legal right to request a patient’s medical information.
- Access computerized patient record and/or other computer systems for patient information needed for faxing and for other requestors of the medical record.
- Performs at established quantitative and qualitative work standards to meet departmental goals and objectives.
- Promotes quality improvement, staff and patient safety, and cultural diversity through departmental operations and by personal performance.
- Provides service in a manner that is appropriate for the patient’s age; demonstrates knowledge and skills necessary to meet the patient’s physical, psychosocial, educational, and safety needs.
- Presents a courteous and helpful demeanor, appropriate for age, to all patients, visitors, other LRHS employees/medical staff, or any other person an employee encounters while representing the health system.
- Maintains current knowledge related to applicable statutes, regulations, guidelines and standards necessary to perform job duties in accordance with the requirements of the Corporate Compliance Plan.
- Understands and follows all hospital and departmental policies and procedures:
- Attends departmental meetings, TJC required workshops, and other in service, seminars as appropriate for job function. Follows hospital safety rules.
- Contributes to the identification and solution of problems. Maintains strict confidentiality of patient and hospital information.
- Scans, quality validation, and verify all hospital documents such as IP, INO, SDC, Outpatient clinical, Clinics, Express Care, RCR, ER, and Loose Sheet scanning of all records the day following the encounter, scanning any loose sheets as they are received.
- Scans within Meditech and archiving Module for documents to be viewed online for patient case
- Verifies and validates image/document errors and makes necessary changes
- Reviews with clinic/department when documents are in an error status and reviews steps to have them sent to HIM correctly
Qualifications:
- Associate degree required. RHIT certification.
- Annual influenza vaccine required.
