Job Description

Niagara Hospice is looking for a full time Medical Social Worker to join our team of compassionate care givers. The Niagara Hospice Medical Social Worker will provides clinical social work services within the scope of practice to patients/families admitted to the Hospice program.

Responsibilities & Specific Duties

  • Assist patient in facilitating their goals; per patient choice
  • Assist patients and families in addressing their anticipatory grief issues as needed and nursing home information as need
  • Provide patients and families with volunteer services information
  • Assist with health teaching/disease progression
  • Conduct suicide prevention/lethality assessment
  • Mediate and advocate for patient choices
  • Assess families for potential complicated mourning, disenfranchised grief, or psychopathological history, and refer for pre-bereavement and /or mental health services as warranted
  • Identify and coordinate a plan to meet the psychosocial needs and services with other patient/family needs and services being addressed by the team
  • Participate in assessments, admissions, discharge planning, service availability, plan design, implementation of services, review of services, and evaluation responsibilities of the team.
  • Provide special awareness and in the identification and dealing of psychosocial stresses of patients and families in accordance with their own cultural and spiritual beliefs as they go through anticipatory grief, the dying process, and death
  • Establish necessary service provider linkages with allied health and social service agencies on behalf of the Hospice patient/family, with appropriate and advocacy
  • Maintain confidential, accurate, timely, legible, detailed, and professionally written records as required by Niagara Hospice and consistent with State, Federal, Third party payor regulations
  • Coordinate delivery of services to maximize effectiveness and eliminate duplication with contracted and/or cooperating social service units of hospitals, DSS, and other public and private agencies
  • Participate in Social Work on-call system.

Qualifications

  • A minimum of a Masters Degree in Social Work (MSW) from a college or university accredited by the Council on Social Work Education required.
  • A minimum of one (1) year experience in a social service agency adhering to acceptable standards
