Job Description

  1. Job Summary

The Neurosurgery Patient Navigator is a member of the healthcare team and assists patients with "navigating" through the healthcare system to obtain timely care. The RN Patient Navigators help coordinate patient care, in conjunction with Neurosurgery clinic team from time of diagnosis through discharge. The RN Patient Navigator is a patient advocate who will work collaboratively with multidisciplinary and medical teams to deliver quality patient care. Monitors patient outcomes, and participates in quality improvement activities. Works alongside management staff to implement and develop established or new programs related to department efficiency and cost-effective program implementation.

  1. Minimum Job Requirements:

Work Experience:

Three years of direct patient care experience in an acute care setting. Previous case management experience or related experience is preferred.

License/Registration/Certification:

Current Texas or compact RN license required.

Current CPR

Education and Training:

A Bachelor's degree in Nursing required. Master's degree in Nursing preferred.

Skills:

    1. Excellent interpersonal communication and negotiation skills.
    2. Ability to perform nursing assessments and patient triage.
    3. Critical thinking skills.
    4. Ability to work autonomously.
    5. Ability to prioritize and reprioritize quickly.
    6. Strong analytical, data management and computer skills.
    7. Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.
    8. Able to work with people of all social, economic, and cultural backgrounds. Culturally sensitive.
    9. Flexible, open-minded and adaptable to change.
    10. Understanding of pre-acute and post-acute venues of care and community resources.
    11. Strong organizational and time management skills, as evidenced by the capacity to prioritize multiple tasks and role components.
    12. Ability to work independently and exercise sound judgment in interactions with physicians, payers, and patients/families.
    13. Bilingual English/Spanish preferred.
    14. Ability to read and comprehend English at a level necessary to follow written and oral instructions and safety precautions.
