We are seeking a warm, relationship-driven Enrollment Coordinator to join our PACE (Program of All-Inclusive Care for the Elderly) team in Wilmington, NC. This is a critical, high-impact role responsible for guiding older adults and their families through the enrollment process into a life-changing care program.

About the Role

As an Enrollment Coordinator, you will be the front line of our program, working with prospective participants, families, community partners, and state agencies to ensure a smooth and compassionate intake and enrollment process.

You will:

  • Build trust with seniors and caregivers during a pivotal life transition
  • Coordinate eligibility and enrollment with DSS and NC Medicaid
  • Represent the PACE program within the community
  • Ensure each applicant receives clear guidance, support, and timely follow-up


Key Responsibilities

  • Manage intake and enrollment process from referral to admission
  • Conduct assessments in homes, hospitals, or community settings
  • Assist with Medicaid eligibility and application/recertification processes
  • Serve as liaison with DSS, healthcare providers, and community agencies
  • Maintain accurate records, documentation, and reporting requirements
  • Educate families on PACE services and alternative resources when needed
  • Build and maintain referral relationships within the community
  • Participate in interdisciplinary team discussions for enrollment decisions

  • Organized and detail-oriented with strong follow-through
  • Skilled at navigating complex systems (Medicaid, healthcare eligibility, etc.)


Minimum Qualifications

  • High School Diploma or equivalent required; Associate’s degree preferred
  • 2+ years of experience in:
    • Healthcare, social services, or case management
    • Medicaid/Medicare, long-term care, or eligibility coordination
  • Experience working with seniors or vulnerable populations preferred
  • Knowledge of Medicaid eligibility and healthcare systems strongly preferred
  • Strong communication, documentation, and organizational skills
  • Valid driver’s license and reliable transportation required


Preferred Experience

  • PACE program knowledge or experience
  • DSS or Medicaid eligibility experience
  • Healthcare intake, admissions, or enrollment coordination
  • Bilingual (Spanish/English) a plus


Work Environment & Expectations

  • Combination of office, field visits, and community outreach
  • Interaction with participants in homes, hospitals, and care settings
  • Fast-paced environment with deadlines and sensitive situations
