Job Description

Description:
  • Performs the duties of a clinical pharmacist including medication management and patient counseling.
  1. Functions under a Collaborative Drug Therapy Agreement to initiate, modify, or discontinue medications for chronic disease states.
  2. Counsel patients, perform medication reconciliation, and provide clinical recommendations regarding medication therapy.
  3. Document appropriately within the electronic health record.
  4. Address prior authorizations, refill requests, and other clinical documents in the electronic health record.
  5. Work under TCCH protocols including, but not limited to, the refill protocol and therapeutic interchange protocol.
  6. Collaborate with staff to provide in-service education related to clinical pharmacy.
  7. Supervise and work collaboratively with clinical pharmacy technicians.
  • Performs the duties of a pharmacist including dispensing prescriptions and counseling patients on their medications.
  1. Maintain accurate documentation in the pharmacy software system.
  2. Assess patient’s medication history.
  3. Review pharmacy profiles per Washington State requirements.
  4. Complete final check of prescriptions appropriately within pharmacy system.
  5. Supervise ancillary pharmacy staff.
  • Clinical and Technical (must be familiar with the following fundamentals/activities):
  1. Utilizes and supervises pharmacy technicians/assistants as appropriate with the pharmacist assuming final responsibility for work performed by the staff. The pharmacist may delegate duties or activities to the staff only as allowed by law and policies/procedures. Provides appropriate instructions/education for pharmacy staff.
  2. Works within a team concept. Exhibits tact and understanding in all interpersonal relations. Demonstrates flexibility, adaptability and good problem-solving skills in a variety of settings.
  3. Assists with training and orientation of new pharmacy employees or students.
  4. Assists TCCH Providers in matters concerning drug therapy as requested.
  5. Fills prescriptions and counsels patients as needed.
  • Complies with clinic policy and procedures consistently in all aspects of employment.
  1. Supports the organization’s mission, policies, and procedures in all aspects of employment.
  2. Participates in peer reviews and other quality improvement initiatives.
  3. Is responsible for knowledge of and compliance with health care and pharmacy department requirements for fire/safety/disasters, hazardous materials, aseptic technique, infection control, use of personal protective equipment and confidentiality issues
  4. Attends all required safety education programs as required
  • Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.
  1. Interacts with all of the above in a considerate, helpful and courteous manner as observed by Department Director and peers.
  2. Fosters mature professional relationships with fellow employees in a courteous and friendly manner as measured by management observation and peer input.

  5. Conducts all work activities with respect for rights and wishes of patients, visitors, families and fellow employees.
  6. Maintains confidentiality of all clinic and patient information at all times as observed by peers and management.
  7. Presents neat appearance in proper attire and identification as required by the position, department and organization policy.
  8. Continuously displays a positive attitude within the department and across departmental lines to contribute to the overall customer service program in place.
  • Promotes effective working relations and works collaboratively within and outside of the department.
  1. Participates in staff meetings as required.
  2. Supports Tri-Cities Community Health’s mission, policies and procedures through attendance and participation at committee meetings, if applicable.
  3. Uses appropriate communication skills with patients and co–workers.
  4. Clarifies instructions and asks for guidance when additional information is needed.
  5. Completes work assignments on time/readily accepts assignments from supervisor.
  6. Reports to work on time and as scheduled.
  • Performs other duties as assigned.

Compensation

The salary range for this position is $127,462 to $185,084 annually. Placement within this range will depend on qualifications and experience.

Requirements:
  • Bachelor of Pharmacy, Doctor of Pharmacy from an accredited school or college of pharmacy. Completion of an accredited Pharmacy Residency preferred.
  • Experience and/or training in clinical pharmacy practice.
  • Washington State Pharmacist License in good standing
