MINIMUM QUALIFICATIONS:
1. Graduate of an accredited school of Practical Nursing.
2. Current licensure in the State of Illinois (or documentation of license pending.)
3. Successful background clearance, physical and drug/alcohol screening
4. A valid Illinois driver’s license and qualify as DCFS licensed driver
PREFERRED QUALIFICATIONS:
1. Previous experience with persons with disabilities preferred.
STANDARD OF PERFORMANCE:
The Licensed Practical Nurse (LPN) provides health services to the students of The Hope Learning Academy Melrose Park.
ESSENTIAL FUNCTIONS:
1. Assists the RN Case Manager in formulating and implementing client health care plans.
2. Evaluates health care practices of the clients. Encourages proper care techniques/practices.
3. Observes, identifies and documents health care needs and problems of clients.
4. Passes medications as required.
5. Correctly prepares field trip medications in a timely manner.
6. Performs nursing treatments, direct care, and other nursing tasks as required.
7. Communicates and documents appropriate information, i.e. new orders, discontinued physician orders, transfers, discharges, admissions, accidents,
incidents, injuries, changes in client conditions, new treatments to be done, problems with taking medications, seizure activity and other pertinent
information.
8. Is responsible for primary clients in terms of ongoing monthly physical assessments and weekly documentation of client's general status.
9. Documents observations, treatments and responses to treatments in a timely manner and in accordance with policy.
10 . Acts as liaison between client, staff and RN Case Manager. Notifies RN Case Manager when appropriate and documents appropriately.
11. Assists with physical examination, procedures or treatments.
12. Carries out physician's orders.
13. Transcribes physician's orders promptly and precisely.
14. Makes rounds on units and/or in classrooms on a regular basis.
OTHER RESPONSIBLITIES:
1. Attends and contributes to staffings, pre-clinics and other meetings involving primary clients.
2. Willingly goes to units where needed to pass medications.
3. Demonstrates skills in interpersonal communications.
4. Maintains professional relationships with other members of the Interdisciplinary Team.
5. Actively participates regularly and actively in assigned committee meetings.
6. Maintains familiarity with current nursing policies and procedures.
7. Follows nursing dress code.
8. Identifies potential safety hazards and reports them and willingly keeps the work area clean, orderly and free of safety hazards.
9. Attends all mandatory in-service meetings.
10. Assumes responsibility as a mandated reporter concerning incidents of purported abuse/neglect of students as outlined in the agency’s policies and procedures.
11. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
