Position Summary

The Program Specialist/Supervisor oversees one or more residential programs and all Quality Management and Incident Management activities.

Compliance

  • Ensures that Individual’s needs are always maintained in accordance with ISP’s.
  • Ensures that all Direct Support Professionals are properly trained in the core curriculum of the Office of Developmental Programs and certified in Medication Administration.
  • Record reviews of all Staff’s training records.
  • Ensures that Chapter 6400 and 6500 homes are compliant with all relevant State, County, and Regional requirements.
  • Completes a regulatory self-assessment of each home the agency operates both under Chapter 6400 and 6500 within 3 to 6 months prior to the expiration date of the agency’s certificate of compliance, to measure and record compliance with this chapter.
  • Completes a quarterly Home Safety Inspection on all Chapter 6400 Residential sites. Submits maintenance reports needed to the Assistant Program Director.
  • Member of the agency’s Safety Committee.
  • Conducts a regulatory record review quarterly of all Individuals receiving service through Laurel House, Inc. Record review shall consist of 3 records per Program Specialist per quarter.
  • Acts as Incident Manager for the agency.
  • Takes training to become a Certified Investigator. Conducts investigations.
  • Responsible for instituting uniform practices for incidents as outlined in the Bulletin such as: building organizational policies, structures, and training to support incident management; timely and appropriate action in response to incidents; reporting of incidents; investigation of incidents; taking corrective action in response to incidents that both mitigate risk and decrease the chance of a future occurrence of a similar incident.
  • Conducts monthly analysis of all incidents and quarterly analysis of incident trends.
  • Schedules quarterly Risk Management/Incident Management meetings to review trends and corrective actions.
  • Conducts Provider Administrative Reviews of all Critical Incidents and enter information into EIM.
  • Facilitates scheduling of Peer Reviews of Certified Investigations.
  • Member of the Human Rights Team (HRT) for Restrictive review.
  • Development of the Quality Management Plan, setting up goals for improvement. Conducting the regulatory quarterly review meetings for updates of progress on goals. Implementing the goals along with Laurel House’s Administrative Management Team.
  • Attends a minimum of 24 hours of in-service training per year.
  • Other duties as assigned.

Programming

  • Identifies Individual skills and strives to support ongoing development and growth. Completes annual assessments, Social Emotional Environmental Plans, needed Back-up Plans.
  • In conjunction with the Program Director, participates in the development of ISPs.
  • Recommends revisions to the ISP as warranted.
  • Informs all Laurel House team members of all ISP meetings.
  • Responsible for the accurate and responsible implementation of Individual ISPs.
  • Assists in the planning and preparation of Agency-based programming initiatives.
  • Identifies and coordinates Individual participation in community events that are of Individual interest.
  • Responsible for reviewing or completing all regulatory forms, and other required reports.
  • Acts as a liaison between the Agency and relevant County and State Agencies.
  • Serves as an Advocate for Individuals to assure rights and choices are safeguarded.
  • Supervises, monitors, and evaluates services provided to the Individual.
  • Counsels Individuals on personal problems and values and offers guidance on making informed decision.
  • Serves as the primary liaison between Individuals, their families, and other significant people in the lives of Individuals.
  • Transports Individuals as needed.
  • Assumes on-call responsibilities as scheduled.
  • Coordinates the training of Direct Support Professionals in the context of the health and safety needs relevant to each Individual.
  • Assigns household maintenance and purchasing tasks to Direct Support Professionals, overseeing their performance to ensure the meeting of Individual needs.
  • Leads regularly scheduled house or staff meetings to address the overall delivery of service and care.
  • Responsible for all Direct Support Professional staff work schedules.
  • Conduct all on site Direct Support Professional and related training.
  • Cooperate with all internal critical incidents.
  • Other duties as necessary.

Essential Functions

  • Must be able to push/pull/lift up to 50 pounds.
  • Must be able to sit/stand/walk for extended periods of time.
