SCHEDULES:
SATURDAY - TUESDAY 11PM -7AM
WEDNESDAY - FRIDAY 11PM - 7AM
JOB SUMMARY: Supporting individuals in independent living situations, remote monitoring allows a degree of independence. This position is utilizing technology to alert direct support professionals when support is needed.
ESSENTIAL JOB RESPONSIBILITIES
Receive and respond to alerts form phones or iPads.
Remain in geographic locations to which wifi and technology does not interfere with alerts and communication
Maintain documentation of supports provided to supported individuals.
Problem solve situations occurring with individuals
Staff would monitor all technology for each apartment and respond accordingly. Such as checking on the person, calling 911 if needed.
Complete all required notes in Therap.
IP’s/6 month reports.
Review of LON’s 30 day prior to IP
Complete Life Skills Assessments 30 days prior to IP
Updating and revising information/programs/IP’s on Therap
Scanning documents/paperwork into individual files on Therap
Update emergency reference forms when needed.
Audit/Review daily notes in Therap for accuracy and completion. Notify Manager with any issues/concerns
Assist with morning routines of each person if needed. Such as cooking, showering, overseeing meds are taken, ensuring properly dressed and morning hygiene (tooth brushing, shaving, hair brushed etc) has been completed
See office area/apartment is clean before leaving.
Order office supplies as needed
Send weekly updates to coordinator of any issues/concerns
Complete monthly attendance.
Update appointment sheets when change in medications
Ensure appointment sheets are filled out for all scheduled appointments.
Monthly appointment audit-review and document any appointments needed and/or labs and results
Complete any delegated tasks from Manager.
MINIMUM QUALIFICATIONS
High school diploma, or equivalent
Valid driver’s license, proof of registration, insurable driving record, and personal vehicle throughout the duration of employment
Food Service License
Basic fluency in oral and written English
Computer/email skills required
ADDITIONAL REQUIREMENTS
Ability to analyze situations develop creative solutions, act quickly and remain calm in a crisis, meet frequent deadlines, handle stressful situations without loss of efficiency and/or composure;
Ability to obtain and utilize CPR, First Aid, PMT, Medication Administration Certification within DDS guidelines and to remain in compliance throughout employment;
2 years’ experience in field of Human Services;
Ability to engage in behavioral interventions and techniques as necessary
Must have the ability to work effectively and harmoniously with others; flexibility with respect to hours of work and changing conditions; be of good moral character; have an interest in the welfare of the individuals; have training and/or interest in the field of developmental disabilities; have a willingness/ability to communicate with and relate to individuals, parents, guardians, fellow staff, representatives of community agencies, and Agency management.
PHYSICAL REQUIREMENTS
• Lifting a minimum of 50 pounds
