As a Home Health Physical Therapist, you will:

• Perform comprehensive initial evaluations to assess patient needs and develop individualized treatment plans.
• Design and implement evidence-based therapy programs to improve mobility, strength, balance, function, and overall quality of life.
• Monitor patient progress, adjust treatment plans, and ensure optimal outcomes.
• Educate patients and family members on exercises, safety techniques, and home programs.
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.
• Maintain accurate, timely clinical documentation in accordance with company policies and regulatory requirements.
• Follow infection control and patient safety standards during every visit.
• Deliver compassionate, patient-centered care while promoting our mission and values.

• Graduate of an accredited Physical Therapy program.
• Current, unrestricted Physical Therapist license in the state of practice.
• CPR Certification.
• Valid driver's license and reliable transportation with current auto insurance.
• One (1) year of Home Health experience preferred.
• Homecare Homebase experience preferred.

• Ability to stand, walk, sit, bend, kneel, squat, crawl, and climb throughout the workday.
• Ability to safely lift, carry, push, and pull up to 100 pounds with or without reasonable accommodation.
