Position Summary

The Field Coordinator is responsible for conducting home visits, supporting caregivers, monitoring participants satisfaction, resolving service concerns, and ensuring service are delivered according to each participant individual care plan. This position serves as a liaison between participants, caregivers, Care Coordination Unit (CCU), Manage Care Organization (MCO), and branch staff.

Reporting Relationship

  • Reports to Branch Manager/Lead Supervisor.

Responsibilities/Activities

  • Conduct initial and ongoing supervisor home visits
  • Review participant care plans and ensure services are delivered as authorized.
  • Monitor participant satisfaction and quality of care
  • Support and coach Home Care Aides in the field
  • Address participant and caregiver concerns promptly and professionally.
  • Document all participant interactions accurately and timely

Qualifications/Education

  • High school graduate or equivalent.
  • Valid driver's license, reliable transportation, and proof of auto insurance Required
  • Abiltiy to work independently
  • Excellent communication and customer service skills
  • Basic computer skills, including Microsoft Office

Preferred

  • Experience in home care, healthcare, social services, case management, or human services
  • Knowledge of Illinois Department of Aging (IDoA) programs
  • Bilingual (English/Spanish) is a plus

Schedule

  • Full-time
  • Monday - Friday
  • Occasional evening or weekends as needed
