Job Description
Summary: Coordinate, monitor and assist all persons determined to be eligible for the program with all aspects of their services. Maintain organized, accurate, complete, and detailed case file records that comply with all organizational and regulatory standards on each person served. Remain up to date and knowledgeable about other resources in the community that might provide individuals with the opportunity to live, learn, work, and play in their communities.
Essential Duties and Responsibilities: include the following items, but other duties may be assigned.
Productivity and Reliability:
- Coordinates the admission of all assigned referrals and provides assistance with procuring required documentation necessary to determine eligibility.
- Responsible for securing required prescription for services to be received as identified in the Individual Treatment Plan (ITP).
- Coordinates the development of the Individual Treatment Plan (ITP) annually.
- With input from person served, stakeholders and other team members, develop long and short-term goals and corresponding objectives that are clear, meaningful, measurable, and consistent with level of development.
- Reviews each ITP and goals/objectives at least quarterly to determine effectiveness and progress.
- Composes a detailed report describing the outcome of each objective no less than quarterly.
- Maintains case files for persons served in compliance with all organizational and regulatory standards.
- Assists individuals in identifying and utilizing community resources and natural support networks by working cooperatively with individuals, families, state and/or federal agencies and other service providers.
- Works with individual served and if applicable the parent/guardian to resolve crises.
- Utilizes positive behavior intervention techniques to manage and promote improved behavior.
- Assists individuals in self-advocacy.
- Facilitates transition of individual from case manager to day program staff when admissions process is complete and provides day program staff all documents necessary to implement ITP of the individual and document progress in goals and objectives.
- Participates in activities that promote community awareness of Easterseals Arkansas programs including attending transition fairs and transition IEP meetings at local schools.
- Conducts and documents monthly contact meetings with each individual on caseload.
- Ensures that the Adult Services databases and other informational tracking systems remain current and accurate.
Other duties as assigned to include but not limited to and are subject to change:
- Complete training upon hire and bi-annually thereafter, to operate an Easterseals vehicle and provide transportation to activities and events.
- Provide classroom and front desk coverage (as needed)
- Provide arrival and dismissal coverage in the afternoon and/or morning (as needed)

Employee has access to all client protected health information within his cohort of individuals served. Employees will not attempt to access any other protected health information unless otherwise approved by Supervisor or Privacy Officer.

Education and/or Experience:
Bachelor's degree (B. A.) preferred from a four-year college or university in Psychology, Social Work, Special Education or related field or a minimum of 2 years equivalent experience working with individuals with developmental disabilities in a DD setting.

- Strong computer skills to use email, calendar, word processor, spreadsheet, video conferencing, and presentation software.
- Strong orientation to deadlines and detail.
- Demonstrated ability to work effectively and collaboratively with individuals at diverse levels.

Certificates, Licenses, Registrations:
High School Diploma
Valid Arkansas Driver's License
