Job Description
Reports To: Unit Director Trainings: Annual, Effective 1/21 BLS Required existing incumbents with Heart Saver First Aid CPR/AED, will be required to renew with BLS, all new hires effective this same date, are required to have BLS within 90 days from date of hire or transfer. CPI required within 90 days of hire or transfer Position Summary: The
Case Manager CCBHC completes all required educational interventions. This position assists in completion of screening process and accurately report information to team members and clients and families.
Duties and Responsibilities: Complete all required educational interventions Assists in the completion of screening form Accurately reports information to team members and participates with team members and clients/families in the completion of a Service Plan within established time frames. Demonstrates knowledge of crisis intervention and behavior management of aggressive behavior. Assists in the implementation of psycho-educational programs. Accurately documents all client interaction within established time frames. Provides services to clients/families in their native language or is able to procure the services of a translator. Seeks supervision in a professional manner and documents supervision. Participates with team members and clients/families in discharge planning. Develops an adequate professional relationship with all referral sources and relevant community agencies. Adequately responds to the needs of clients, families and referral sources. Provides core services to clients as assigned As appropriate to scope of practice, assists in the data collection for each assigned client, including the following: The relationship of each clients physical status to dependence. The nature of the comparison to use alcohol and/or other drugs. The intensity of the clients mental preoccupation with using alcohol and/or drugs. Establish and maintain positive community supports Maintain compliance with the DMHAS licensing regulations including the confidentially of drug and alcohol clients Participate in treatment team conferences to support best practices Models and supports clients and staff in health and wellness activities and with using healthy coping skills.
Licensure/Certification: Current and valid NJ drivers license
Minimal Qualifications: Education: Bachelors degree in behavioral health field required. Experience: At least one year experience in behavioral health field or appropriate internship required. Understands and supports a trauma-informed system of care. Experience with illness management a& recovery (IMR) and motivational interviewing (MI) preferred. Excellent computer skills required. Strong verbal and written communication skills required. Self-directed and flexibility required to meet the needs of administration and management. Sensitivity to a cultural diverse client population and workplace required. Candidates must support and validate individual differences to create a harmonious work environment.
