Job Summary

Under general supervision, provides a wide variety of professional level of nursing duties for patients; acts as case manager in the coordination of services and personnel needed to execute the plan of treatment for each patient and family receiving home health care.

Qualifications

Education

• Required
  • High School Diploma, GED, or equivalent
  • Associate’s degree in Nursing
  • Completion of an accredited Nursing program

Experience:

• Required
  • One year of experience as a Registered Nurse (RN)
• Preferred
  • One year of experience in Home Health

Certification/Licensure:

• Required
  • Registered Nurse (RN), with authorization to practice in the State of Texas
  • Basic Life Support (BLS) Certification
    • Must obtain within ninety (90) days of start date
  • Valid Driver’s License
  • Auto Liability Insurance
  • Reliable transportation
