Job Description
The Licensed Clinical Social Worker provides comprehensive clinical and psychosocial services to individuals and families living with or affected by HIV/AIDS. Services include individual and group therapy, psychosocial assessments, crisis intervention, care coordination, referrals and linkages to community resources, and ongoing supportive services. The Social Worker also contributes to community outreach, mental health education, awareness initiatives, and community events designed to reduce stigma and increase access to HIV/AIDS and behavioral health services.
The Social Worker works collaboratively with an interdisciplinary team and provides person-centered, trauma-informed, culturally responsive, and strengths-based care. The position requires demonstrated comfort and skill working with older adults and an understanding of the unique psychosocial, behavioral health, and social service needs associated with aging with HIV/AIDS and other chronic health conditions.
Requirements:RESPONSIBILITIES:
1. Provide individual and group therapy, supportive counseling, and other clinical interventions to JPA patients and individuals living with HIV/AIDS and their families, addressing behavioral health, emotional, interpersonal, and psychosocial needs.
2. Conduct comprehensive psychosocial and behavioral health assessments, including social histories, to identify patient strengths, needs, barriers to care, treatment goals, and the need for additional psychiatric, psychological, or specialized services.
3. Develop and implement individualized treatment and service plans and provide ongoing clinical support, follow-up, and reassessment based on patient needs and progress.
4. Provide crisis intervention and emergency psychosocial support while assessing safety, responding to urgent needs, and coordinating appropriate clinical and community-based services.
5. Provide care coordination, advocacy, referrals, and linkages to medical, behavioral health and other community resources to address social determinants of health and barriers to care.
6. Provide person-centered services to older adults living with HIV/AIDS and other chronic health conditions, addressing concerns related to aging, social isolation, caregiving, grief and loss, financial and housing stability, healthcare access, and maintaining independence.
7. Collaborate with medical providers, behavioral health professionals, case managers, community-based organizations, and other members of the interdisciplinary team to coordinate integrated, patient-centered care.
8. Participate in community outreach, health fairs, educational programs, and community events while promoting mental health awareness, HIV/AIDS education, wellness, and stigma reduction among patients, families, and community members.
9. Maintain timely, accurate, and clinically appropriate documentation, including psychosocial assessments, treatment plans, progress notes, referrals, case records, reports, caseload activity, and required statistics, in accordance with agency and regulatory requirements.
10. Participate in interdisciplinary meetings, professional development, quality improvement initiatives, and emergency/on-call services as required.
11. Assist with other special projects and assignments as assigned.
MINIMUM QUALIFICATIONS:
1. Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE), with current New York State licensure as a Licensed Clinical Social Worker (LCSW).
2. New York State licensure or eligibility for appropriate social work licensure in accordance with applicable regulations.
3. Experience providing clinical or psychosocial services to individuals living with HIV/AIDS, individuals with behavioral health needs, or other medically complex populations preferred.
4. Experience providing individual and/or group therapy, psychosocial assessment, crisis intervention, care coordination, and community-based services preferred.
5. Demonstrated ability to work effectively with older adults and address the psychosocial needs associated with aging and chronic health conditions.
6. Strong communication, documentation, advocacy, and interdisciplinary collaboration skills.
7. Commitment to trauma-informed, culturally responsive, person-centered, and strengths-based practice.
8. Bilingual English/Spanish preferred
