Pay Range: $25.00 - $31.50 per hour
Pay Transparency:
The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey.
The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
Job Overview:
The Patient Navigator will provide warm hand-off to providers; escort patients to appointments; provide patient education; and maintain contact with patients throughout their care, coordinate internal and external referrals for prevention, support and wellness services, specifically integrated screenings and provide continuous support for all clients who have been tested for HIV regardless of test result.
Qualifications:
Required:
- High School Diploma or equivalent
- Bi-lingual, and able to read, write and speak Spanish proficiently required,
- Experience in a health care setting highly desirable
- Standard office equipment (PC, copy machine, fax, binding machine, calculator)
- Proficient in Microsoft Word, Excel and PowerPoint
Preferred:
Associate’s degree preferred
Scheduling Requirements:
- Shift- 8:30am-4:30pm
- Monday-Friday
- Full Time
Essential Functions:
- Advocate for clients who have legal benefit concerns
- Assess client needs, prepare appropriate individual service plans and maintain records
- Assist clients in implementing the service plan by networking and referral, internally and/or to other community resources
- Assist homeless or precariously housed clients towards more stable housing
- Assist with insurance and benefit applications
- Attend and participate in staff meetings and supervisory conferences
- Conduct referral and linkage activities to housing, food programs and other customer needs
- Coordinate with social services and insurance companies or other financial services to ensure ability to pay for care
- Develop and maintain database of professional contacts
- Ensure confidentiality of all records
- Establish, maintain and update client case records using approved agency forms and procedures
- Maintain and update client information in specific database
- Maintain working relationships with other community providers
- Prepare monthly reports in accordance with agency policies and procedures
- Provide relevant literature to clients and members of the community
- Provide written reports and maintain statistics as required
- Represent the agency at community events, meetings, and conferences
