About Kids Home Health Care is seeking an RN Case Manager to provide skilled nursing care and coordinate home health services for pediatric patients and their families.

This hybrid position combines working from home with in-person patient visits. Complete case management, care coordination and documentation remotely, and visit patients in their homes for assessments, skilled nursing care and supervisory visits. Flexible hours allow you to organize your workday around patient visits, care coordination and administrative responsibilities.

You will work closely with physicians, caregivers, therapists and clinical leadership to develop individualized care plans and support safe, coordinated care in the home.

Under the supervision of the Clinical Supervisor and Director of Nursing, you will:

  • Assess patients and coordinate admissions. Complete comprehensive assessments to identify nursing needs and determine appropriate services, including admissions in Colorado Springs and Pueblo.
  • Develop and manage care plans. Partner with caregivers and physicians to establish, implement and update individualized plans of care based on physician orders and patient needs.
  • Provide skilled nursing care. Deliver services requiring specialized nursing knowledge and clinical judgment, following established procedures and the patient’s plan of care.
  • Monitor progress and respond to changes. Evaluate the effectiveness of care, identify changes in condition, and promptly communicate concerns to the physician and clinical supervisor. Obtain and process updated orders as needed.
  • Complete clinical documentation remotely. Accurately document visits, assessments, physician orders, admissions, transfers, recertifications, resumptions of care and discharges within agency timelines. Complete OASIS assessments when applicable.
  • Coordinate services across the care team. Collaborate with case managers, therapists, social workers and outside providers. Communicate with involved disciplines and agencies at least every 30 days and document care coordination according to agency policy.
  • Arrange additional services. Identify needs for therapy, social work or paraprofessional support and coordinate referrals with the physician and appropriate supervisor.
  • Supervise LPNs and CNAs. Complete and document supervisory visits, evaluate the care provided, and contribute to required performance evaluations according to agency policy and applicable requirements.
  • Educate caregivers and clinical staff. Provide instruction and support to promote safe, consistent care. Delegate responsibilities appropriately within your scope of practice.
  • Plan for transitions and discharge. Coordinate discharge planning with families and the care team, and provide the physician with a discharge summary when services end.
  • Maintain clear communication. Promptly report patient incidents, complaints, incomplete assignments and schedule changes. Keep patient coverage and daily visit reporting current.
  • Participate in clinical teamwork. Attend weekly case management meetings, complete required education and maintain clinical skills through professional development.
  • Manage your schedule effectively. Balance remote responsibilities and field visits, complete timely documentation, and participate in assigned on-call coverage and holiday visits when needed.
  • Follow professional standards. Perform responsibilities in accordance with physician orders, agency policies, professional nursing standards and applicable requirements.

Bring your nursing expertise to a hybrid role offering flexible hours, a salary of $83,000–$87,000 annually, and the opportunity to support children and their families at home.

Requirements:

Candidates should have an active RN license authorizing practice in Colorado, strong clinical assessment and organizational skills, and the ability to work independently while communicating effectively with families and the care team. Reliable transportation and the ability to travel between patient homes and service areas are necessary.

Work takes place from home and in patients’ homes, with travel for field visits. Patient care involves a high risk of exposure to blood and bodily fluids and possible exposure to pets, pet dander and other environmental allergens.
