POSITION TITLE: | Family Health Program Coordinator |
POSITION SUMMARY: | The Family Health Program (FHP) Coordinator supports the delivery and coordination of comprehensive sexual and reproductive health services within the primary care setting. This role combines program coordination, patient care support, community outreach, staff education, and administrative responsibilities to ensure high-quality, patient-centered care. Working collaboratively with providers, nurses, medical assistants, and administrative staff, the Program Coordinator helps advance the Health Center's mission while promoting equitable access to sexual and reproductive health services. |

PRIMARY RESPONSIBILITIES:
- Coordinate the Family Health (FH) Program
- Supports collaboration of medical and reception staff involved in the FH program.
- Maintains and revises medical and administrative forms, patient education materials, and job aids as needed.
- Manages monthly reporting to Massachusetts Department of Public Health and other funders.
- Assists in yearly workplan development and goal setting for the FH program.
- Prepares reports and statistics of program activities as needed.
- Support Community Outreach and Health Promotion
- Provides culturally appropriate and medically accurate health information to the communities served by the Health Center and priority populations in community-based settings.
- Coordinates community collaborations with partner organizations such as schools, shelters, youth serving agencies, social service agencies, and community centers.
- Maintains relationship with local domestic violence programs and rape crisis centers.
- Provide Administrative and Patient Support
- Provides guidance to team members and serves as a resource for information related to the FH program.
- Assists with scheduling appointments for follow up care as needed.
- Clearly documents all phone calls, interactions with patients, and attempts to contact patients in a timely manner.
- Coordinates care as directed via phone, patient portal, and/or letters as needed.
- Monitors FH task group and responds in a timely manner.
- Train and Mentor Staff
- Participates in student, medical assistant, nurse, provider, and community health worker new hire orientation as needed.
- Collaborates with supervisor and new learner to review the progress of the new learner toward meeting learning objectives.
- Helps create an environment conducive to learning. This includes a friendly environment for the new learner to ask questions and report and correct mistakes made.
- Demonstrates clinical skills while explaining clinical reasoning and decision-making.
Assists with competency assessment and sign-off.
- Support Policy and Workflow Development
- Collaborates with FHP team to develop, review, and update protocols.
- Establishes workflows for medical and reception teams.
- Creates necessary training around protocols.
- Provide Clinical Support
- Conducts pre-visit planning activities for the assigned patient panel. Participates in daily team huddles with provider to prepare for clinic when needed.
- Participates in Patient Center Medical Home (PCMH) by reviewing charts for health screenings due.
- Prepares exam room for patient visits, restocks and cleans as needed.
- Calls patients from the waiting room to the exam room using the proper patient identifiers and pronouns.
- Obtains the chief complaint, vital signs, allergies, surveys, screenings, consent forms (if applicable), and performs a medication reconciliation. Initiates visit documentation.
- Provides patient education as appropriate.
- Collects lab samples, obtains an EKG (if deemed competent) as needed. Administers all CLIA waived lab tests and medical tests (e.g. HCG, UA, urine drug screen, ear flush, hearing and vision screenings) per clinical protocols.
- Acts as a chaperone for medical exams as needed.
- Prepares the provider for in-office procedures (e.g. Pap test, mole removal, IUD and implant placement and removal).
- Administers vaccines as directed by primary care provider.
- Coordinates in-clinic services between providers, the lab, MAs, and nurses, to assist with timely patient flow.
- Understands and maintains standard precautions in all clinical activities, uses PPE appropriately.
- Demonstrates teamwork and works to build cohesive, professional relationships with all CHCFC staff and patients.
- Maintains current CPR certification.
- Assists in emergencies as trained, call for outside assistance as directed.
- Communicate Effectively Across Teams
- Communicates effectively with patients and interdisciplinary staff to promote coordinated, patient-centered care.
- Shares pertinent information with providers and care team members in a timely manner.
- Maintains professional, respectful communication across departments.
- Participates in team meetings and quality improvement initiatives as appropriate.
REQUIRED SKILLS, EDUCATION & EXPERIENCE:
Education: Associate's degree in health sciences, public health, human services, or a related field preferred; equivalent education and experience will be considered.
Proficiency in written and spoken English. Bi-lingual English/Spanish, English/Haitian Creole, English/Moldovan, or English/Russian a plus.
One year of experience in reproductive health, community health, primary care, or a related healthcare setting preferred.
Skills:
- Demonstrated interest in reproductive health and justice.
- Demonstrated ability to work effectively with people of diverse backgrounds, identities, cultures, languages, and lived experiences.
- Ability to efficiently operate basic office equipment (multi-line phone, fax, photocopier)
- Ability to use Microsoft Office Suite (Word, Excel, and Outlook) and Internet.
- Understands and is familiar with basic medical terminology.
- Excellent interpersonal and communication skills
- Strong organizational and time-management skills
- Ability to maintain confidentiality and HIPAA compliance
- Experience using an electronic health record (Epic preferred)
- Ability to work independently and collaboratively

WORKING CONDITIONS:
While performing the required duties of this job, the employee is exposed to weather conditions prevalent when traveling between Health Center locations where medical care is provided. This position performs tasks that involve exposure to blood, body fluids or tissues.
