You build relationships, develop individualized programs, support personal goals, and help residents live active, fulfilling lives with as much independence as possible.As a QIDP, you will work closely with residents, families, and an interdisciplinary team of professionals to understand each resident's needs, interests, and goals. You will help create opportunities for residents to grow physically, mentally, emotionally, and spiritually while supporting their individual path toward greater independence.Experienced QIDPs are welcome. - Oversee resident programs, goals, activities, and community outings
- Develop and implement individualized programs based on each resident's needs, abilities, interests, and goals
- Support residents in developing skills that promote greater independence
- Work with residents to identify and pursue meaningful personal goals and interests
- Collaborate with the interdisciplinary team to develop, monitor, and adjust resident programs
- Help ensure residents receive quality care and services that meet their individual needs
- Advocate for the rights, choices, dignity, and well-being of residents
- Provide leadership and direction to staff supporting resident programs
- Communicate effectively with residents, families, coworkers, and other professionals
- Manage time and resources effectively while meeting deadlines and program requirements
- Participate in community activities and outings that promote inclusion and independence
- Provide physical care and support to residents as needed
- Adapt programs and approaches based on residents' changing needs and abilities

- Bachelor's degree required, preferably in human services or a related field such as Psychology, Sociology, Special Education, or Rehabilitation
- At least 1 year of experience working with people with developmental disabilities
- Strong written and verbal communication skills
- Demonstrated leadership ability
- Ability to work effectively as part of a team
- Commitment to protecting and promoting the rights of residents
- Strong organizational and time-management skills
- Ability to manage resources and meet deadlines
- Reliability, flexibility, and adaptability
- Ability to provide physical care to residents
- Ability to work a variety of shifts and hours and supervise staff as required
- Valid Illinois Driver's License preferred
PayBase Pay
- Case Manager/QIDP: $25.50 per hour
Shift Differentials
- PM Monday-Friday: +$1.50 per hour
- Weekend: +$6 per hour
Potential Pay Range: $25.50-$31.50 per hourBenefits
