The case manager (CM) serves as a member of the multi-disciplinary mental health team responsible for providing recovery-oriented services to clients with severe and persistent mental illness, who may also present with co-occurring physical and/or developmental disabilities, and/or substance abuse issues. The CM will provide services related to the client’s transition from nursing home and long-term care facilities, into their own apartments through the Clustered Housing Program, providing support related to linkage to services, benefits and entitlements, as needed per client’s service/treatment plan.

Essential Responsibilities, include but are not limited to:
- Review/educate clients in areas identified in the service/treatment plan, such as medication management, healthy lifestyle, psychosocial needs, independent daily living skills, and self-management skills.
- Maintain efficient and consistent communication with the Clustered Housing Program Manager and Housing Counselor.
- Provide clients with referral, linkage, and follow-up services to needed resources, such as housing, entitlements, educational, vocational and guardianship services.
- Provide information to outside agencies as requested to coordinate and assure the appropriateness of services.
- Serve as liaison with client’s managed care companies and health providers to ensure services and medications are authorized.
- Provide support with scheduling and coordinating client’s medical appointments based on client’s medical needs, being physically present at the appointments, and providing transportation if/when needed.
- Provide brief, supportive goal directed counseling to clients who may be experiencing difficulties with their transition into the community and/or based on assessed needs.
- Develop and facilitate therapeutic/recreational activities on site and in community.
- Attain/maintain assigned completing all documentation on a timely basis, including clinical documentation, and service reporting.
- Conduct assessments, to determine need/appropriateness for services, coordinating linkage to services identified.
- Assist Program Manager with program development tasks such as designing workflows and procedures manuals.
- At all times maintain confidentiality according to the Illinois Mental Health code and Confidentiality Act.
- Responsible for participating in required in-service training and community training seminars at the discretion of the Director.
- Be prepared to present information on outside training to clinical staff.
- Shift: 8am-4pm Monday to Friday.

Requirements:EDUCATION, SKILLS, and/or EXPERIENCE
Bachelor’s degree in social work, counseling, psychology or related field or 5 years’ experience in the field of Mental Health. Must have minimum of one-year experience working within the social service arena, including internships. At least one-year case management experience, working with diverse populations with multi-faceted needs. Experience with mental health populations and knowledge of Medicaid rules preferred. Must have excellent written and verbal communication skills, decision making and problem-solving abilities, and must have exceptional computer and organizational skills. Individuals must be able to work autonomously. Valid driver's license and approval through agency's insurance carrier to drive agency's vehicles required.
Must pass Criminal Background Check and Nurse’s Aide Registry if applicable.
