JOB SUMMARY:
A Community Health Worker in a hospital supports patients and families by addressing social determinants of health, improving care transitions, and reducing barriers that effect recovery and follow‑up care. This role works closely with case management, nursing, physicians, and social services to ensure patients have the resources and support needed for a safe discharge and successful recovery at home or in the community.
QUALIFICATIONS:
  • High school diploma or equivalent
  • Experience in community outreach, social services, or healthcare preferred
PHYSICAL REQUIREMENTS:
  • Able to speak clearly and distinctly with staff, physicians, patients and families.
  • Able to extensively utilize telephone for communication.
  • Demonstrates adequate physical and emotional stamina to deal with stressful situations and to complete the necessary work schedule.
  • Able to sit/stand/walk for extended periods of time throughout the work schedule, with some light lifting and bending.
  • Demonstrates adequate vision, or correctable with glasses/contacts, for reading and preparing written documents.
  • Must have adequate perception of sounds or adequate hearing with corrections.
  • Able to hear spoken communications from staff, physicians, patients, and families as well as pages.
  • Demonstrates adequate manual dexterity to prepare written documents.
  • Able to work in a latex burdened environment.
