We are searching for a Certified Peer Support Lead to join our newest team at the Women's Housing Navigation shelter in Albuquerque, New Mexico!


OBJECTIVE AND POSITION SUMMARY
The CPSW Lead / Community Support Worker (LLA Case Manager) plays a dual role in supporting CPLC’s mission to deliver trauma-informed, client-centered housing and supportive services. This position combines leadership responsibilities at the Women’s Housing Navigation Center with case management duties under the Local Lead Agency (LLA) program. The CPSW Lead provides mentorship, oversight, and support to shelter-based housing staff while also delivering individualized case management to clients referred through the LLA system. This integrated role enhances both shelter operations and community-based services, promoting housing stability, life skills development, and long-term self-sufficiency for clients.

SUPERVISORY RESPONSIBILITIES:

  • Provides guidance and mentorship to the Housing Support Specialists and Certified Peer Support Workers at the Women’s Shelter
  • Coordinates team huddles, case staffing, onboarding, and compliance support

SPECIALIZATIONS

  • Trauma-informed case management and peer support
  • Knowledge of housing programs, tenancy supports, and landlord relations
  • Strong understanding of service navigation and social determinants of health
  • Crisis de-escalation, problem-solving, and systems collaboration
  • Skilled in LLA reporting, documentation, and eligibility tracking
  • Leadership and coaching within shelter settings

PRIMARY DUTIES AND ESSENTIAL FUNCTIONS

CPSW Lead Responsibilities

  • Lead daily team huddles and facilitate communication across staff
  • Provide mentorship, supervision, and guidance to shelter case managers and peer staff
  • Monitor caseload distribution and ensure timely service delivery
  • Assist with complex client cases and help identify housing solutions
  • Support onboarding and training for new team members
  • Maintain and update compliance reports and team performance dashboards
  • Participate in multidisciplinary meetings and case conferencing
  • Serve as liaison between program staff and leadership for operational updates
  • Document team activities and assist with program evaluations
  • Assist in the creation and development of policies and procedures ensuring alignment with contractual obligations

LLA Case Management Responsibilities

  • Conduct intake and assessments to determine client needs and eligibility
  • Develop individualized service plans focused on housing stability, employment, and life skills
  • Maintain regular home visits and provide independent living skills instruction
  • Respond to landlord and property manager concerns to preserve housing
  • Maintain the LLA waitlist and document all client interactions in compliance with LLA requirements
  • Facilitate referrals to community partners and wraparound services
  • Support client efforts in housing searches and maintain housing retention
  • Host quarterly stakeholder and client meetings for the LLA program
  • Collaborate with community service providers and internal teams for coordinated care
  • Perform other duties as assigned to support the mission of the shelter and CPLC

MINIMUM QUALIFICATIONS AND COMPETENCIES

Education /Background:

  • Certified Peer Support Worker required
  • A Bachelor’s degree, preferred or
    • A combination of a minimum of six years of work experience and education will be considered
    • Minimum two years of experience in housing, case management, or social service delivery
  • Previous experience working with individuals experiencing homelessness or in a supportive housing setting is highly desirable
  • Strong understanding of trauma-informed care principles and a commitment to applying them in practice
  • Ability to work effectively with diverse populations and demonstrate cultural competency
  • Excellent communication, problem-solving, and organizational skills
  • Ability to maintain professionalism and confidentiality in a fast-paced, high-stress environment
  • Proficiency in basic computer applications, including Microsoft Office and client management software
  • Valid driver’s license and ability to travel locally as needed
