LVN Hospice Nurse, Part Time at Acacia Hospice of Southern California LLC – Irvine, California

Responsibilities:
• Hospice visits - daytime visits
• Provide accurate and timely documentation
• Assess and provide patient and family/caregiver education
• Perform other duties as assigned by the registered nurse
• Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc.

Qualifications:
• Demonstrated compassion and commitment to patient-centered care
• Active LVN license in the state of California
• Valid California driver’s license and reliable transportation
• Minimum 1 year of hospice nursing experience; strong understanding of hospice philosophy and end-of-life care
• Prior experience in nursing or related medical fields
• HomecareHomebase/Pointcare EMR: 1 year (Preferred)

Job Types: Part-time

• Weekdays

Service Coverage:
• Orange County

Compensation: $30.00 - $35.00 per hour.
