Job Description
The Behavioral Health Care Coordinator performs a dual function role that integrates front office operations with clinical support and care coordination activities to ensure efficient clinic flow and high-quality patient care. This position is responsible for managing patient access functions, including scheduling, registration, and payment collection, while also supporting behavioral health care coordination services in alignment with organizational protocols. The Coordinator serves as a key point of contact for patients, facilitating communication between patients, providers, and community resources, and supporting continuity of care through referral coordination, service navigation, and patient engagement in treatment plans. The Behavioral Health Care Coordinator applies knowledge of behavioral health processes, confidentiality standards, and healthcare systems to ensure accurate documentation, regulatory compliance, and effective collaboration with interdisciplinary teams, contributing to both operational efficiency and positive patient outcomes.
Responsibilities:
- Communicate effectively with corporate management, administrative personnel, and medical staff across the health system.
- Schedule patient appointmentsin accordance withdaily provider quotas, ensuring efficient appointment management.
- Answer phone calls, check inpatients,and notify clinical staff of their arrival.
- Collect fees for servicesrenderedandassistin checking patients out.
- File and organize patient charts, ensuring proper documentation and easy retrieval.
- Confirm patient appointments for the following day as assigned.
- Conduct Care Coordinationin accordance withthe PanCare Coordination program.
- Attend,facilitateand supportin-school basedservicesin Bay District Schools.
- Serve as the primary point of contact for patients, coordinating care across multiple providers, including therapists, physicians, and community services.
- Advocate on behalf of patients to ensure they receivetimely, appropriate care, and represent their needs and concerns with healthcare providers, insurers, and other relevant parties.
- Connect patients to necessary resources, such as therapy, support groups, financialassistance, and housing, ensuring they have the tools needed for their recovery.
- Provide emotional support, education, and guidance to patients and their families, helping them understand behavioral health conditions, treatment options, and coping strategies.
- Maintainaccurate, detailed, and up-to-date records on patients' cases, includingappointment conditions, demographic updates, communication with providers, and any changes in status.
- Provideconnection with providers to offercrisis intervention and stabilization support when needed, ensuring the safety and well-being of the patient during emergency situations.
- Collaborate with community-based organizations and resources, ensuring that patients are connected to services that complement their treatment plans.
- Assistin processing and sending patient chart information to theappropriate entities, ensuring all required authorizations are obtained, documented, and filed.
- Submit record requests to other entities with properly executed patient authorization forms, documentingactionsand filing requests in the patient’s chart.
- Assistin verifying patient eligibility for sliding scale fees, ensuring proper documentation.
- Balance the day sheet and manage cash, checks, and credit card receipts.
- Keepan accuratedaily postage log.
- Assistinmaintainingan efficient and smooth clinic front office, performingadditionaltasks as needed, as directed by theFront Office Supervisorand/orChief Behavioral Health Officer.
- Support patient billing tasks as assigned by the Front Office Supervisor and/or Chief Behavioral Health Officer.
- Escalate complex scheduling, patient concerns, and unresolved issues to theFront Office Supervisorand/orChief Behavioral Health Officerorleadership perprotocol.
- Maintain regular and predictable attendance to support clinic operations.
- Other duties as assigned.
Education/Experience:
- High School Diploma or GED; somecollege ispreferred.
- Aminimum of6monthsexperience in a customer service positionrequired; previousexperience in a healthcare administrative environment preferred.
- Customer Service: Proven experience ina customerservice or patient-facing role,demonstratingexcellent communication skills and a professional demeanor.
