Job Description
Job Summary
The Health Care Coordinator (HCC) is responsible for overseeing and coordinating the medical needs of individuals with intellectual and developmental disabilities (IDD). This role ensures that all health-related services are delivered in accordance with physician orders, regulatory requirements, and organizational standards. The HCC works collaboratively with the local nurse, Clinical Services Administrator, physicians, and direct care staff to maintain continuity of care, ensure compliance, and promote optimal health outcomes.
Essential Duties & Responsibilities
Manage and coordinate the medical care of approximately 40–50 individuals.Serve as the primary liaison between physicians, the local nurse/Clinical Services Administrator, and the interdisciplinary team regarding all medical needs and changes in condition.Ensure physicians are consistently updated on participants’ medical status and emerging concerns.Submit required monthly medical monitoring reports accurately and timely.Communicate all changes in physician orders promptly to the local nurse/Clinical Services Administrator, team members, and direct care staff.Complete monthly health status and medication reviews to ensure accuracy and compliance.Ensure all medical appointments are scheduled, attended, and properly documented.Maintain accurate, current electronic health records, including uploading all required medical documentation.Collaborate closely with the local nurse/Clinical Services Administrator to address concerns, clarify orders, and ensure appropriate follow-up.Conduct routine audits and reviews of health-related documentation, protocols, and compliance standards.Monitor medication regimens to ensure they are administered as prescribed and remain current.Ensure proper medication storage, organization, and destruction in accordance with policy and regulations.Track and monitor medical needs as directed by the Clinical Services Administrator or Quality Assurance (QA) Specialist.Maintain awareness of individual-specific parameters for physician notification and act accordingly.Coordinate and lead post-hospitalization meetings, ensuring appropriate follow-up care is implemented.Conduct home visits to verify medication organization, documentation accuracy, and overall health record management.Ensure continuity of care by coordinating coverage and availability of backup HCC support.Apply the core principles of effective healthcare coordination: observation, communication, documentation, and action.Other duties as assigned.
Education & Experience
Must have a valid driver’s license and clear driving recordCurrent automobile insuranceAbility to pass registry and background checksOne year of experience working with individuals with developmental disabilities or caring for a person in a healthcare setting
Knowledge, Skills & Abilities
Basic understanding of medical terminology.Ability to read, interpret, and follow physician (MD) orders, with appropriate consultation from the local nurse/Clinical Services Administrator when needed.Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.Strong organizational skills with high attention to detail.Effective time management and ability to prioritize tasks in a fast-paced environment.Ability to maintain accurate records and ensure compliance with regulatory requirements.Must maintain confidentiality and protect individual rightsAbility to project a caring and professional image at all timesProven skills in communication, observation, problem solving, and time management

Ability to communicate with other people in-person, by telephone, and in writingAbility to use verbal and physical intervention to redirect and/or restrain a person who is physically aggressive or self-abusiveAbility to safely provide supervision and monitoring to meet the needs of people supported by Hands of HeartlandAbility to perform repetitive tasks for extended periods of time to include but not limited to bending, stooping, sitting, and reaching Ability to lift up to 50 pounds as needed
