About This Position
We are seeking a dedicated and detail-oriented Clinical Coordinator to support the daily clinical and operational functions of our CHAP-accredited home health agency. This is primarily an in-office position working closely with both the Director of Nursing (DON) and the Administrator to ensure clinical excellence, regulatory compliance, and smooth coordination of patient care services.
This role is ideal for a Licensed Practical Nurse (LPN), Registered Nurse (RN), or a highly experienced non-clinical professional with strong home health operations experience. The right candidate will be organized, proactive, and comfortable operating in a compliance-driven, fast-paced healthcare environment. You will play a key role in supporting quality outcomes, documentation standards, survey readiness, and day-to-day clinical operations.
Key Responsibilities
• Work collaboratively with the DON and Administrator to support clinical and operational workflows
• Coordinate and track clinician schedules, visits, Start of Care, recertifications, and follow-ups
• Assist with intake, referrals, and patient onboarding coordination
• Review clinical documentation for accuracy, completeness, and timeliness
• Support CHAP, CMS, and state compliance standards and ongoing survey readiness
• Assist with chart audits, quality assurance, and performance improvement activities (QAPI support)
• Track physician orders, plans of care, and ensure timely follow-through
• Communicate with nurses, therapists, aides, physicians’ offices, and office staff to ensure continuity of care
• Maintain organized and compliant clinical records in the EMR system (e.g., Axxess or similar)
• Help identify documentation or workflow issues and escalate to DON/Administrator as appropriate
• Support onboarding and orientation coordination for new clinical staff (as needed)
• Serve as a liaison between clinical staff and administration
Qualifications
• LPN or RN license in good standing OR significant non-clinical home health operations experience
• Home health experience strongly preferred
• Strong understanding of home health workflows, documentation, and compliance requirements
• Familiarity with OASIS, Plans of Care, physician orders, and survey readiness is a strong plus
• Experience with EMR systems (Axxess or similar preferred)
• Strong organizational, time management, and multitasking skills
• High attention to detail with a strong compliance mindset
• Excellent communication skills (written and verbal)
• Comfortable working in-office and collaborating closely with leadership
• Professional, dependable, and solutions-oriented
