  • POSITION SUMMARY

    • The Team Lead Care Coordinator provides direct Care Coordination services while serving as a point person for complex cases, mentorship, and departmental support. This licensed clinical role maintains a caseload but also assists in managing high-acuity patients, placement barriers, length of stay challenges, and departmental workflow needs. The Team Lead supports the Manager of Care Coordination in daily operations and helps foster a culture of collaboration, accountability, and excellence in patient-centered care.

  • POSITION QUALIFICATIONS
    • Minimum Education
      • Bachelor of Social Work or Nursing

    • Minimum Experience
      • 5 years nursing or medical social work.

    • Required Registration/License/Certification
      • Active Alabama license by State Board of Social Work or Registered Nurse

    • Preferred Education
      • MSW or MSN

    • Preferred Experience
      • 8 years of nursing or social work experience in case management or medical social work

    • Preferred Registration/License/Certification
      • Case Management Certification from an Accredited Organization
      • Licensed Independent Clinical Social Worker (LICSW)

    • Other Requirements
      • N/A
