Job Overview:
As a Social Worker, you will provide frontline services to patients with conditions spanning the entire health care continuum.
The goal for this acute care, nonclinical position is to discharge patients home or to another safe environment.
As an essential member of the interdisciplinary team, you will work in concert with doctors, nurses, and allied health professionals to sensitize other health care providers to the social and emotional aspects of a patient's illness.
Utilizing your case management skills, you will help patients and their families address and resolve the social, financial and psychological problems related to their health condition.
Qualifications:
Required:
- Master's degree in Social Work (M.S.W.) from an accredited School of Social Work
- New Jersey State Social Work Licensure (L.S.W. or L.C.S.W.)
- Good communication and writing skills
- Ability to work in a multidisciplinary team
Preferred:
- Bilingual in English and Spanish
- Hospital work experience
Certifications and Licenses Required:
- New Jersey State Social Work Licensure (L.S.W. or L.C.S.W.)

Essential Functions:
- Understands the financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication
- Collaborates closely with physician advisor
- Participate in interdisciplinary rounds and facilitate communication with the interdisciplinary care team
- Complete psychosocial assessments and implement patient-centered care plans that meet needs identified in assessments
- Discharge planning activities: seamless and timely coordination of care including: referrals to home care, DME, SNF, rehab placements, hospice, community resources, and transportation arrangements
- Collaborates with RN Case Manager in all discharge planning
- Collaborate with department leadership to streamline processes and implement strategies to create more efficient delivery of care,
- Ensures compliance with regulatory documentation standards
