Description: Summary

The High-Risk Breast Cancer Nurse Practitioner is a specialized member of our healthcare team, dedicated to the expert management of individuals at increased risk for breast cancer. This advanced practice role emphasizes comprehensive risk assessment, genetic counseling, and the development of tailored surveillance and risk-reduction strategies. Practitioners in this position provide advanced, evidence-based care, serve as clinical leaders within multidisciplinary teams, and guide patients and families through complex decisions regarding hereditary risk and cancer prevention. This position offers the opportunity to build meaningful relationships with high-risk patients, work collaboratively with oncology, genetics, and surgical colleagues, and lead efforts to advance hereditary breast cancer prevention and early detection in our community. It requires a strong background in oncology, hereditary cancer syndromes, and high-risk patient management.

Key Clinical Responsibilities

* Lead comprehensive risk assessments and genetic counseling for individuals with a personal or family history of breast cancer, hereditary cancer syndromes, or other high-risk factors. Evaluate personal and family medical histories to stratify risk and guide management.

* Develop, implement, and monitor individualized high-risk surveillance and risk-reduction care plans. Prescribe and manage preventive and endocrine risk-reducing therapies based on evolving evidence and patient risk profiles.

* Serve as the primary point of coordination between breast surgeons, medical oncologists, radiologists, genetic counselors, and other specialists to ensure seamless, multidisciplinary care for high-risk patients.

* Deliver targeted education and counseling to patients and families on hereditary breast cancer risk, tailored surveillance strategies, lifestyle modifications, and available preventive interventions.

* Oversee all aspects of high-risk clinic follow-up, including ongoing surveillance scheduling, timely referrals for advanced imaging or specialty consultation, and structured survivorship care for patients with elevated breast cancer risk.

* Order, interpret, and act upon advanced laboratory and imaging diagnostics (including breast MRI, mammography, and genetic testing panels) as clinically indicated for high-risk populations.

* Provide expert clinical guidance to high-risk patients and triage staff, ensuring rapid response to patient concerns, abnormal findings, and urgent needs associated with hereditary breast cancer risk.

* Communicate complex genetic testing results, imaging findings, and pathology reports to patients and their families with clarity and sensitivity, supporting informed decision-making in high-risk scenarios.

* Partner with high-risk patients to co-create evidence-based, risk-adapted management plans that align with personal values, preferences, and goals for cancer prevention and early detection.

* Champion a multidisciplinary approach by collaborating with oncology, surgery, genetics, imaging, and support services to optimize care for individuals at increased genetic or familial risk.

* Maintain meticulous documentation of risk assessments, care plans, patient communications, and outcomes specific to high-risk breast cancer management.

Requirements:

Required Qualifications

* Minimum 2-3 years of experience working as a WHNP in a clinical setting preferred, with demonstrated competency in providing comprehensive women’s health services.

* Graduated from an accredited school of nursing.

* Current, unrestricted Minnesota RN and Women’s Health Nurse Practitioner (WHNP-BC) licensure, current CPR/BLS certification, and DEA registration (or ability to obtain).

* Demonstrated ability to understand, adhere to, and contribute to the development of established policies, procedures, and evidence-based clinical protocols.

* Proven ability to work collaboratively with physicians, nurses, medical assistants, and administrative staff in a busy and complex environment, demonstrating tact, diplomacy, professionalism, and emotional intelligence.

* Strong charting/documentation skills.
