MDS Coordinator Position Summary
The MDS Coordinator is responsible for managing the full assessment, care planning, and reimbursement documentation process for all residents within the Skilled Nursing Facility. This position oversees the timely and accurate completion of all Minimum Data Set (MDS) assessments in compliance with federal and state regulations, supporting reimbursement under Medicare, managed care, and other applicable insurance models.
The MDS Coordinator serves as the facilitys subject matter expert on MDS regulations, CMS requirements, PDPM methodology, and assessment timelines while providing leadership and supervision to the Clinical Review Nurse / MDS Nurse (LPN). This role ensures interdisciplinary documentation accuracy, coordinated admissions review, survey readiness, and strong clinical integrity across the Skilled Nursing Facility.
Key responsibilities include:
- Overseeing the complete Resident Assessment Instrument (RAI) process to ensure full regulatory compliance
- Maintaining the master assessment calendar for OBRA, PPS, and insurance-driven assessments
- Ensuring accurate clinical coding and documentation to support reimbursement and quality outcomes
- Reviewing assessments for completeness prior to submission and resolving discrepancies with the interdisciplinary team
- Evaluating resident documentation to confirm skilled coverage criteria for Medicare and managed care reimbursement
- Collaborating with billing, finance, and administrative teams to ensure assessment-driven reimbursement accuracy
- Conducting routine audits of clinical records, therapy documentation, and nursing notes to validate billing integrity
- Leading Care Area Assessments (CAAs) and supporting individualized, current care plans reflective of resident needs
- Facilitating collaboration across nursing, therapy, dietary, social services, and providers for consistent documentation inputs
- Participating in care plan meetings to review resident progress, discharge planning, and ongoing skilled needs
- Monitoring Quality Measures (QMs), Five-Star indicators, and other MDS-driven reports to support QAPI efforts
- Supporting facility readiness for state survey and CMS audits through ongoing documentation review and staff coaching
- Providing oversight of clinical review for incoming admissions to ensure appropriate Skilled Nursing placement
- Educating staff on documentation practices that impact MDS accuracy, reimbursement, and compliance
- Participating in leadership meetings and operational initiatives as assigned by the Senior Director of Health Services
Qualifications and Requirements
- Current and active Registered Nurse (RN) license in the State of Florida (preferred)
- Minimum of 35 years experience in Skilled Nursing or Long-Term Care
- Minimum of 2 years MDS/RAI experience strongly preferred
- Strong working knowledge of PPS, PDPM, OBRA, Medicare, and managed care documentation requirements
- Excellent organizational, leadership, and interdisciplinary coordination skills
- High attention to detail, audit capability, and ability to meet strict deadlines
- Proficiency with electronic medical records and clinical documentation systems
- Ability to maintain confidentiality and uphold HIPAA and ethical standards
- Physical ability to meet the demands of the role, including standing, walking, lifting, and assisting residents as required
