JOB SUMMARY

The Diabetes Education Program Coordinator is responsible for overseeing the daily operation of the DSME program. He/she ensures that the National Standards (NSDSME) are met and maintained at all times. Roles and responsibilities include:
-
Oversees the planning, implementation and evaluation of the DSME program
-
Counsels patients with diabetes and their support persons in appropriate self-care skills that promote the healthiest possible lifestyle for management of this chronic disease.

Arranges and coordinates the activities of the Advisory Board
-
Serves as a liaison between the staff, Advisory Board, other departments and administration
-
Facilitates staff education concerning diabetes through scheduled educational offerings that support evidence based practice
-
Responsible for maintaining ADA Recognition and participating in the evaluation of the program's effectiveness
-
Upholds Major Hospital's and Rampart Rural Health Clinic's policies and procedures

MINIMUM QUALIFICATIONS
-
Professional & Technical Skills
-
Basic computer skills and teaching ability.

Education
-
Graduate from an accredited School of Nursing or Health Education (BSN preferred)
-
Knowledge of patient education practices and principles
-
-
License(s) or Certification(s)
-
Certified Diabetes Educator or willing to seek certification
-
Current Indiana License as a Registered Nurse
-
CPR Certification
-
-
Other Skills or Requirements
-
Ability to communicate effecively

Customer service oriented personality

Good organizational skills

Skilled to Care for Certain Age-Related Patient Groups (incumbents will be skilled in the care of the following patient groups)
