Position Summary
SmithLife Homecare is seeking a compassionate and experienced Registered Nurse (RN) to join our team on a PRN (as-needed) basis. In this flexible role, you will provide skilled nursing care to clients in their homes, helping to ensure their safety, comfort, and independence.
The Registered Nurse (RN) provides skilled nursing assessment, care planning, and clinical oversight to clients receiving services through SmithLife Homecare on a PRN (as-needed) basis. The RN is responsible for developing, implementing, and supervising the Plan of Care, ensuring services are delivered safely, effectively, and in compliance with all applicable Maryland COMAR and Washington, DC Home Support Agency regulations.
The RN plays a critical leadership role in supporting care transitions, managing changes in condition, and guiding caregivers and LPNs in the home setting.
Qualifications
- Active Registered Nurse (RN) license in good standing in Maryland and/or Washington, DC
- Graduate of an accredited school of nursing
- Minimum of three (3) years of clinical nursing experience preferred (home care, hospital, or community-based)
- Experience with geriatric populations and chronic disease management strongly preferred
- Demonstrated ability to work independently and make sound clinical decisions
- Strong assessment, care planning, and documentation skills
- Current CPR/BLS certification required
Core Responsibilities
Clinical Assessment & Care Planning
- Perform comprehensive nursing assessments in the home setting
- Develop, implement, and update individualized Plans of Care
- Evaluate client needs, risks, and goals to support safe aging in place
- Conduct reassessments, including change-in-condition visits and routine supervisory visits
Clinical Oversight & Delegation
- Supervise and provide clinical oversight to LPNs, CNAs, and caregivers
- Delegate nursing tasks in accordance with state regulations and scope of practice
- Ensure all care provided aligns with the Plan of Care and agency standards
- Provide guidance, training, and support to field staff
Care Coordination & Communication
- Collaborate with care coordinators, physicians, families, and interdisciplinary teams
- Communicate changes in condition and recommendations for care adjustments
- Support hospital-to-home transitions and other care transitions
- Serve as a clinical resource for staff and families
Direct Skilled Nursing Care
- Provide direct care as needed, including but not limited to:
- Medication management and administration
- Wound care and infection prevention
- Chronic disease monitoring and management
- Patient and caregiver education
Documentation & Compliance
- Complete all documentation in WellSky in accordance with agency policy and regulatory requirements
- Ensure documentation is accurate, timely, and audit-ready
- Maintain compliance with Maryland COMAR and Washington, DC regulations
- Adhere to all SmithLife Homecare policies and procedures
Quality & Safety
- Identify and respond to changes in client condition, including escalation to physician or emergency services when appropriate
- Maintain a safe and therapeutic care environment
- Report incidents, concerns, and risks per agency protocol
- Participate in QAPI and quality improvement initiatives
Client & Family Support
- Educate clients and families on care plans, medications, and disease processes
- Provide emotional support and guidance to families navigating care needs
- Promote dignity, independence, and person-centered care
Physical Requirements
- Ability to lift, transfer, and assist clients (50+ lbs as needed)
- Frequent standing, walking, bending, kneeling, and reaching
- Full use of hands and fine motor skills for clinical tasks
Work Environment
- Client homes, including private residences and senior living communities
- Exposure to varying home environments and conditions
- Travel required between client locations
Additional Expectations
- Maintain professionalism, confidentiality, and clinical integrity at all times
- Demonstrate reliability and responsiveness for PRN scheduling needs
- Participate in required trainings and competency validations
- Support agency initiatives, including dementia care best practices (e.g., Positive Approach to Care® principles)
Reports To
- Director of Nursing (DON) or Designee
Schedule & Status
- PRN / Per Diem (As Needed)
- Variable hours based on client needs, including potential evenings, weekends, or short-term assignments
Compensation
- Competitive hourly rate based on experience
- Opportunities for additional hours based on performance and availability
