Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team in Lake Placid, NY. This role involves coordinating and providing support services to individuals and families in our permanent supported housing program who are facing various challenges, including behavioral health issues, addiction, child welfare concerns, unemployment, and housing needs. The ideal candidate will have a strong background in social work or related fields, with experience in crisis intervention and management. The Case Manager will play a vital role in connecting clients with community resources, developing personalized service plans, and ensuring clients receive the appropriate support to achieve stability and well-being.

Responsibilities

  • Conduct comprehensive assessments to identify clients’ needs and develop tailored service plans.
  • Provide crisis intervention and crisis management to clients experiencing urgent needs.
  • Coordinate services related to child welfare, housing supports, Medicare, and workforce development programs.
  • Monitor and mentor clients through various stages of stabilization processes.
  • Collaborate with healthcare providers, social service agencies, and community organizations to ensure seamless service delivery.
  • Maintain accurate documentation of case progress, service plans, and client interactions in compliance with agency policies.
  • Advocate for clients’ rights and assist them in navigating complex systems such as Medicaid or child welfare services.
  • Monitor client progress regularly and adjust service plans as needed to meet evolving needs.
Requirements:
  • Strong crisis intervention and crisis management skills essential for handling urgent situations effectively.
  • Experience in behavioral health, social work, or related fields preferred.
  • Supervising experience or mentoring skills that support client development.
  • Knowledge of child welfare systems, housing programs, Medicare, and workforce development initiatives.
  • Excellent communication skills for engaging clients empathetically and collaborating with multidisciplinary teams.
  • Ability to manage multiple cases efficiently while maintaining attention to detail.
