Crouse’s Pharmacy team is hiring a Medication Reconciliation Coordinator to accurately collect and entering patient medication histories into the hospital's information system for utilization by the Physician responsible for admitting, discharging, or releasing the patient from the hospital. Also responsible for relaying any issues that cannot be addressed by the assistant to the pharmacist.
Pay Range: $20.00 - $34.80 per hour
Available Schedule(s):
- Per diem – scheduled as needed, 10:30am-9:00pm & other shifts as needed, would rotate holidays and weekend coverage
Medication Reconciliation Coordinator Responsibilities:
- Collects and accurately enters a list of the patient's current home medications into the hospital computer system in assigned patients.
- Is responsible for obtaining additional medication information from outside sources when necessary or practical (including physician's office, mail-order pharmacy, retail pharmacy, extended care facility, or another acute care facility).
- Effectively communicates with other healthcare professionals to maintain adequate workflow and identify patient specific needs (i.e. language barriers).
Medication Reconciliation Coordinator Requirements:
- High school diploma or equivalent
- National certification (CPhT) is required to be completed within 2 years from the date of hire. Once obtained, national certification must be maintained throughout employment.
- Minimum of 3 years previous Retail or Hospital Pharmacy experience (which must include consistent order entry)
- Must be able to pass a departmentally administered clinically based medication reconciliation exam with a minimum grade of 70%.
